Objective: Parkinson's disease(PD)is a kind of serious neurodegenerative disease,which is common in the middle-aged and the elderly.Up to now,there is no way to cure PD completely.Therefore,PD treatment is mainly aimed at delaying the progress of disease and reducing the severity of symptoms.Drug therapy is the main treatment for PD patients,but there are adverse reactions,and the effectiveness of improving gait will gradually decline with the progress of the disease.As an effective supplement,surgical treatment is expensive and has a narrow range of indications.At present,sports training has gradually become a research hotspot,among which,Baduanjin training is a complete set of Qigong exercises based on the theory of traditional Chinese medicine,which has a history of more than 800 so far,and is very suitable for the elderly and the weak.In this experiment,Baduanjin training was conducted in early PD patients,by comparing the gait parameters and the scores of anxiety and depression scale of PD patients before and after training,we can explore the influence of Baduanjin training on the gait parameters and emotional disorders of early PD patients,so as to provide new ideas for the sports therapy of Parkinson's disease.Materials and Methods: Twenty PD patients who were followed up in the Department of Neurology of the First Affiliated Hospital of Dalian Medical University from March 2018 to September 2018 were selected.The selection criteria were:(1)Patients with primary Parkinson's disease that meet the diagnostic criteria of the International Movement Disorders Association(2015 edition);(2)HY stage 1-2.5;(3)only treated with levodopa preparations and dopamine receptor agonists;(4)Nosystematic exercise training.Exclusion criteria:(1)those who could not complete the exercise due to illness or combined diseases;(2)those who combined with other Parkinson's disease medications or adjustments during training;(3)those with clear cognitive impairment.Before training,the general data(gender,H-Y grade,age,height,weight,course of disease)of the patients were improved,and the clinical data related to PD were evaluated.For example,Movement Disorder Society-Unified Parkinson's Disease Rating Scale-part III(MDS-UPDRS-III)?Hamilton depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA),using the German Zebris FDM-T gait analysis system evaluates the gait parameters of patients,including the percentage of supporting phase,the percentage of double supporting phase,the percentage of swinging phase,step length,stride,pace,step time,stride time,step width,step frequency,gait line length,forward and backward displacement and lateral symmetry,etc.Results:1.The MDS-UPDRS-? ?HAMA and HAMD scores of early PD patients after training were all lower than those before training,and the differences were statistically significant(P values are P<0.001?P=0.002?P=0.026).2.After early PD training,the percentage of left and right foot support phases and the percentage of double support phases decreased compared with before training,and the percentage of left and right foot swing phases increased compared with before training.The difference was statistically significant(P values are P<0.001?P=0.02?P=0.002?P<0.001?P=0.025).3.After training,the left and right foot step length?stride length and velocity of early PD patients increased before training,and the differences were statistically significant(P values are P=0.004?P=0.025?P=0.011?P=0.002);there was no significant difference in left and right foot stride width?single step time?stride time and cadence before and after training(P values are all greater than 0.05).4.There was no significant difference in gait line length ? forward and backward displacement and lateral symmetry of early PD patients before and after training(P values are all greater than 0.05).Conclusion:1.Baduanjin training can improve the overall movement of early PD patients.2.Baduanjin training can restore the gait stability of early PD patients.3.Baduanjin training can improve the walking ability of early PD patients.4.Baduanjin training had no significant effect on the plantar pressure center transfer in early PD patients during walking.5.Baduanjin training can improve anxiety and depression in early PD patients.
