Objective:In recent years,China's economy has developed rapidly,and people 's lifestyles and quality of life have changed significantly,The incidence and mortality of coronary heart disease and diabetes have increased year by year.Studies have confirmed that atherosclerosis is the pathological basis of coronary heart disease,and chronic inflammatory reactions plays an important role in the occurrence and development of atherosclerosis,Coronary heart disease combined with diabetes may have a more severe inflammatory response.Insulin resistance is the common pathogenesis of coronary heart disease,diabetes,and obesity,It can exist for many years before coronary heart disease,diabetes,and obesity.Plasma complement C1q/tumor necrosis factor-related protein-12(CTRP12)is a new adipokine,which regulates inflammatory response,improves insulin resistance,lowers blood glucose and other biological functions.This study compares the concentration of plasma CTRP12 in the control group,coronary heart disease group,and coronary heart disease combined with type 2 diabetes mellitus and its relationship with the degree of coronary stenosis,in order to provide new ideas for the diagnosis and treatment of coronary heart disease,especially patients with coronary heart disease and type 2 diabetes.Methods:According to the inclusion and exclusion criteria,Patients who were admitted to the Department of Cardiology for chest tightness and chest pain,were planned to undergo coronary angiography(CAG)or percutaneous coronary intervention,In Subei People's Hospital of Jiangsu Province from August 2018 to November 2019,According to whether the patients had type 2 diabetes and coronary angiography,A total of 146 patients were divided into(1)control group(n = 28)(2)coronary heart disease group(n = 54)(3)Coronary heart disease combined with Type 2 diabetes group(n = 64),Subjects were scored by the coronary artery Gensini scoring system,which was used to assess the severity of coronary artery stenosis,TyG index was used to assess the degree of insulin resistance,The concentration of plasma CTRP12 in fasting plasma in the morning was detected by enzyme linked immunosorbent assays(ELISA),Simultaneously Detecting the total cholesterol(Tollycholesterol,TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),High-density lipoproteincholesterol(HDL-C),fasting plasma glucose(FPG),glycated hemoglobin,Hb A1c)and other relevant indicators,Collecting relevant data such as gender,age,height,weight,body mass index(BMI),smoking history,history of high blood pressure,(HHBP)etc,EXCEL,SPSS 22.0,Graphpad Prism 5.0 software were used for analysis the differences among each group,correlation between plasma CTRP12 and Gensini score of coronary stenosis,multiple linear regression analysis was conducted on the factors which affected plasma CTRP12,receiver operating characteristic curve(ROC)was used to analysis plasma CTRP12 to predict the risk of coronary heart disease.Results:1.Fasting plasma glucose(FPG),glycated hemoglobin(Hb A1c),total cholesterol(TC)and TyG index in the CHD + T2 DM group were higher than those in the CHD and CON groups,and the results were statistically significant(P<0.05),The numbers of white blood cells(WBC)?neutrophils(NE)and monocytes(MONO)in the CHD + T2 DM group were higher than those in the CON group,and the results were statistically significant(p <0.05),The numbers of monocytes(MONO)in the CHD group were higher than those in the CON group,and the results were statistically significant(p<0.05),There were no significant differences in gender(Gender),age(Age),smoking history(Smoking),history of hypertension(HHBP),body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),triglyceride(TG),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C),The numbers of lymphocyte among the three groups(p > 0.05).2.The concentration of plasma CTRP12 was the lowest in the CHD + T2 DM group,followed by the CHD group,and the highest in the CON group.The results were statistically significant(P <0.05).Conversely,the Gensini scorein the CHD + T2 DM group was higher than in the CHD group,and the results were statistically significant(P<0.05).3.Univariate correlation analysis showed that the TG,TyG index of the control group was negatively correlated with the plasma CTRP12 concentration,which was statistically significant(P <0.05).Age,BMI,SBP,DBP,TC,HDL-C,LDL-C,FPG,Hb A1 c,WBC,NE,LY,MONO had no correlation with plasma CTRP12 concentration(P> 0.05).The TG?FPG?Hb A1c?TyG and coronary Gensini scores in the case group were negatively correlated with the concentration of plasma CTRP12,which was statisticallysignificant(P<0.05).MONO was positively correlated with the plasma CTRP12 concentration,which was statistically significant(P <0.05).Age,BMI,SBP,DBP,TC,HDL-C,LDL-C,WBC,NE,LY have no correlation with plasma CTRP12(P> 0.05).4.Multiple linear regression analysis showed that the Gensini score and TyG index were independent factors affecting plasma CTRP12,and the results were statistically significant(P <0.05).5.The ROC curve suggested that the area under the curve(AUC)was 0.820 and the95% CI was 0.733 ~ 0.907(P<0.001).The optimal cut-off value for CTRP12 diagnosis of coronary heart disease was 8.40 pg/ ml.Conclusions:1.Compared with the control group,The concentration of plasma CTRP12 decreased in both coronary heart disease and coronary heart disease combined with type 2 diabetes,and was lowest in the coronary heart disease combined with type 2 diabetes.2.The concentration of plasma CTRP12 is negatively correlated with the degree of coronary artery stenosis in patients with coronary heart disease and coronary heart disease combined with type 2 diabetes,that is,the higher the coronary Gensini score,the lower the plasma CTRP12 concentration.3.TyG index value increased in coronary heart disease and coronary heart disease combined with type 2 diabetes,and was inversely proportional to the concentration of plasma CTRP12.
