Objective: To analyze the clinical data of patients with early and locally advanced buccal mucosal cancer,analyze the survival time and recurrence of patients,analyze the relevant factors affecting the survival prognosis and recurrence of patients with early and locally advanced buccal mucosa,and explore the suitable for our patients Treatment mode.Materials and methods: From January 2007 to December 2016,162 patients with buccal mucosa cancer(27 patients in early stage I-II and 135 patients in advanced stage III-IV)who received comprehensive surgerybased comprehensive treatment were selected and applied SPSS 20.0 Statistical software analyzes the data,Kaplan-Meier method for survival analysis,Log-Rank test for single factor analysis,Cox regression model for multi-factor analysis,and correlation analysis for influencing factors.Results: The median overall survival time(mOST)of the patients was 59.0(95% CI: 43.810 ~ 74.190)months,and the median progression-free survival time(mPFS)was 40.0(95 % CI: 25.897 ~ 54.103)months.LogRank test single factor analysis suggested that the factors related to the prognosis of patients with buccal mucosa cancer were: T stage,N stage,buccal muscle invasion,extracapsular invasion,presence of cancerous tissue residue,and peripheral nerve infiltration.Multivariate analysis of the Cox regression model suggested that T stage,N stage,buccal muscle infiltration,and envelope invasion were independent prognostic factors for patients with buccal mucosa cancer.The survival time of lymph node dissection in patients with early buccal mucosal cancer was statistically significant,P value <0.05.The recurrence rates of patients with locally advanced disease who underwent unilateral I-III,unilateral I-IV,unilateral IV,and bilateral neck dissection were 48.4%,65.5%,52.2%,and 50.0%,P>0.05;the median survival time between postoperative radiotherapy and postoperative chemoradiotherapy was 36.0 VS 46.0(P> 0.05).Patients with poor prognosis benefit from radiotherapy and chemotherapy,median OS: 49 months vs 14 months(P > 0.05).Conclusions: 1.The independent prognostic factors for patients' overall survival are T stage,N stage,buccal muscle infiltration,extracapsular invasion.2.Neck dissection of patients with early buccal mucosal cancer may have survival benefits.3.There was no statistically significant difference in the recurrence rates of patients with locally advanced disease in unilateral I-III,unilateral I-IV,unilateral I-V,and bilateral neck clearance.4.Compared with postoperative radiotherapy,locally advanced buccal chemoradiotherapy has no obvious survival benefit.Patients with high-risk factors may have survival benefits after chemoradiotherapy.
