Objective:To investigate the clinical diagnostic values of CD64 index,soluble triggering receptor expressed on myeloid cell-1(s TREM-1),C-reactive protein/albumin(CRP/Alb)ratio,and their combined detection in postoperative infection of colon cancer,which will provide a theoretical basis for the prediction of postoperative infection clinical diagnosis in colon cancer.Methods: 91 patients with colon cancer who underwent elective surgery from January 2015 to January 2018 were selected,they were divided into infection group(38 cases)and non-infection group(53 cases),40 healthy people who underwent physical examination at the same time were selected as control group.On the morning of the second day after operation,the healthy persons and the morning patients' veinal blood were divided into two parts,one part was centrifuged at 3000 R / min for 20 min,and the supernatant was collected and stored in the refrigerator at-80 ?.Another PE tube with diamine tetraacetic acid dipotassium anticoagulant was added,reversed and evenly mixed.Special flow tube was taken,and anti-human CD64 antibody was added to each tube in turn.Flow cytometer was used for detection,and the detection was completed within 2 hours to ensure the accuracy of the results.The levels of serum C-reactive protein(CRP)and albumin(ALB)were detected by enzyme-linked immunosorbent assay(ELISA),and the level of soluble trigger receptor expresses on myeloid cells-1(s TREM-1)were measured by enzyme-linked immunosorbent assay(ELISA).At the same time,all the clinical records of the patients were collected completely.the CD64 index,the level of soluble trigger receptor expresses on myeloid cells-1(s TREM-1),and C-reactive protein/albumin(CRP/Alb)ratio were observed in three groups,the diagnostic values of serum CD64 index,s TREM-1,CRP/Alb ratio,and combined detection of them for postoperative infection of colon cancer were evaluated by ROC.All the data were analyzed by SPSS 17.0,and the data of normal distribution were expressed by mean ± standard deviation(±s).T test was used for comparison between the two groups,ANOVA was used for multiple groups,and binary was used for comparison Logistic regression analysis was used to evaluate the influencing factors of postoperative infection in patients with colorectal cancer,to obtain logistic regression equation,and to obtain a group of new variables PRE-1;and P<0.05 was considered statistically significant.xResults:(1)There was no significant difference in CD64 index,s TREM-1,CRP,ALB and CRP/Alb levels between the control group and the non-infection group(q = 1.761,0.971,1.767,2.392,1.639,P > 0.05);The levels of CD64 index,s TREM-1,CRP and CRP / ALB in the infected group increased significantly(q = 29.315,18.207,42.858,43.853,P < 0.05),and ALB decreased significantly(q = 7.343,P < 0.05);Compared with control group and non-infection group,the CD64 index,the levels of s TREM-1,CRP,and CRP/Alb in infection group increased significantly(P<0.05),while the level of Alb decreased significantly(P<0.05);Compared with the non infection group,the levels of CD64 index,s TREM-1,CRP and CRP / ALB in the infection group were significantly higher(q = 29.506,18.446,43.933,45.119,P < 0.05),and the levels of Alb were significantly lower(q = 5.469,P < 0.05).Multivariate logistic regression analysis showed that CD64 index,s TREM-1 and CRP/Alb levels were all risk factors for postoperative infection of colon cancer;(2)the ROC curve showed that the area under the curve predicted by CD64 index was 0.859(sensitivity was 88.7%,specificity was 86.8%);the area under the curve of s TREM-1 level for predicting postoperative infection of colon cancer was 0.906(sensitivity was 92.5%,specificity was 89.5%);the area under the curve of CRP/Alb level for predicting postoperative infection of colon cancer was 0.894(sensitivity was 88.7%,specificity was 92.1%);the area under the curve of the three combine detection for predicting postoperative infection of colon cancer was 0.978(sensitivity was 94.7%,specificity was 96.2%).Conclusion:CD64 index,s TREM-1 and CRP / ALB levels are all the influential factors of postoperative infection in patients with colon cancer.The combined detection of CD64 index,s TREM-1 and CRP / ALB can improve the diagnosis effect of early postoperative infection of colon cancer patients,which has important clinical value.The combined diagnosis can improve the sensitivity and specificity,which is worthy of clinical reference.
