Objective To evaluate the diagnosis value of Soluble triggering receptor-1 myeloid cells innpatients with sepsisMethods Select 30 cases (sepsis group) who onset of sepsis within 48 hours in our hospital ICU from October 2009 to February 2010 and the same period of the systemic inflammatory response syndrome (SIRS) patients 10 patients (control group). By enzyme-linked immunosorbent assay (ELISA) detection of the two groups into the ICU sTREM-1 concentration in peripheral blood, while one-way immune diffusion method using blood C-reactive protein (CRP) concentration. Using receiver operating characteristic curve (ROC) evaluation of the indicators of sepsis diagnostic accuracy.Results:(1) Peripheral blood sTREM-1 levels of patients with sepsis(44.40±50.43pg/ml) was significantly higher than patients with SIRS (15.28±4.65pg/ml), P= 0.012; (2)the best critical value of sTREM-1 in peripheral blood to diagnosis of sepsis is 17.27pg/ml, sensitivity 60.9%, specificity 75%; and (3) the establishment of ROC curve, into the ICU when the diagnosis of sepsis, peripheral blood sTREM-1 area under the curve of the ROC (AUC) to 0.625 (95% CI:0.403-0.847), and CRP of AUC0.793 (95% CI:0.632-0.955) compared P<0.05.Conclusion:peripheral blood of sTREM-1 and CRP have diagnostic value on early sepsis, but CRP has a greater diagnostic value. Detected by ELISA when each group into the ICU patients sTREM-1 levels, while one-way immune diffusion method using peripheral blood CRP levels.Results(1) Selected 30 cases of sepsis patients,24 patients got better during hospitalization survival (survival group), six cases of death in patients with disease progression (death group), severe sepsis group 1 died,5 died in septic shock group, total mortality was 23.34%; (2) survival group patients sTREM-1 concentration (57.77±57.48 pg/ml) were higher than those (15.28±4.65pg/ml), P=0.002; (3) peripheral blood concentrations of sTREM-1 between Sepsis group, severe sepsis and septic shock group showed:septic shock group (17.78±5.89pg/ml) sTREM-1 levels than severe sepsis group (47.82±47.07 pg/ml) and sepsis group (42.75±37.78 pg/ml) significantly decreased, and there is significant difference, P<0.01, but severe sepsis group and sepsis group had no statistical differences, P>0.05;(4) sTREM-1 concentration and the APACHEⅡscore was negatively correlated with correlation coefficient in sepsis, r=-0.093, P=0.636; Subgroup analysis showed that sTREM-1 concentration and the APACHEⅡscore was negatively correlated in patients with sepsis group r=-0.595, P=0.091; group sTREM-1 levels correlated with APACHEⅡscore inpatients with severe sepsis, correlation coefficient r= 0.530, P=0.177; sTREM-1 levels negatively correlated with APACHEⅡscore in patients with septic shock, Correlation coefficient r=-0.600, P= 0.208。Conclusion:sTREM-1 concentration in peripheral blood can be used as an indicator of prognosis in septic patients, Different stages of sepsis, blood sTREM-1 concentration and the APACHEⅡscore associated with different, dynamic monitoring also may be useful for evaluating patients with sepsis and severity of the disease process Objective:Observed at Characteristics of TCM indifferent stages of patients with sepsis, sTREM-1 and the relationship between TCMMethods:Select onset of sepsis within 48 hours in our hospital ICU were admitted from October 2009 to February 2010,30 patients were divided into three groups according to severity, sepsis group, severe sepsis and septic shock group group all 10 cases. sTREM-1 levels were detected by ELISA when each group into the ICU patients, meantime use of Chinese medicine clinical look, smell, questioning the basic methods, record the symptoms, tongue, pulse, septic shock, according to TCM syndrome differentiation criteria for TCM analysis to determine the patients syndromes。Results:(1) In 30 cases sthenia syndrome was 19 patients (63.3%), asthenia syndrome was 11 cases (36.7%), Which empirical evidence of the rising evil heat in 7 cases,6 cases of phlegm resistance card, blood stasis syndrome in 6 cases; deficiency in 7 cases of qi deficiency, blood deficiency 3; Yang Deficiency 1 case; (2) 9 cases of sthenia syndrome sepsis group,1 case of asthenia syndrome; severe sepsis group sthenia syndrome in 6 cases,4 cases of asthenia syndrome; evidence of septic shock group 3 cases of sthenia syndrome,7 cases of asthenia syndrome; (3) sthenia syndrome group and asthenia syndromegroup sTREM-1 was:45.35±37.27,15.68±7.89 (pg/ml);empirical evidence of the rising evil heat, dampness resistance card, blood stasis syndrome sTREM were 43.6±36.57,46.38±29:8,47.38±29.8 (pg/ml),45.38±29.8,Between the two groups P>0.05; asthenia syndrome Group qi deficiency, blood deficiency, yang deficiency sTREM were 13.47±9.34,15.76±10.5,13.12 (pg/ml),Between the two groups P>0.05。Conclusion:Different stages of sepsis, the distribution of different TCM,Mainly for the asthenia syndrome of TCM in septic patients, STREM-1 concentration in peripheral blood decreased may performance asthenia syndrome。...
