Objective:On this study,the value of TSH combined with Tg in the diagnosis of benign and malignant thyroid nodules is inferred by analyzing the concentration of TSH and Tg in serum of thyroid patients before operation,so as to better differentiate and diagnose the nature of thyroid nodules and reduce the operation rate of benign nodules in clinical work.Methods:The clinical data of 145 patients with thyroid diseases who were admitted to the First Affiliated Hospital of Wannan Medical College for surgical treatment from August 2018 to August 2019 were collected.Patients with previous history of thyroidectomy,Hashimoto's thyroiditis,thyroiditis,hyperthyroidism,hypothyroidism,thyroid fine needle puncture before operation and other basic diseases were excluded.The collected patient data included the basic information of the patient,preoperative imaging,preoperative serology,postoperative routine pathological report and operation method.According to routine pathology,the patients were divided into two groups: benign nodule group(39 cases)and malignant nodule group(106 cases).SPSS software was used to analyze the collected information statistically,and the results of TSH and Tg in diagnosing benign and malignant thyroid nodules were obtained.Results:1.The TSH level(2.21 ± 1.64)mIU/L of patients with thyroid malignant nodules is higher than that of patients with thyroid benign nodules(1.68 ±1.21)mIU/L,it has statistical significance(p<0.05).2.The Tg level of patients withthyroid malignant nodules(66.08±20.21)ng/ml is higher than that of patients with benign nodules(47.32±19.26)ng/ml,it has statistical significance(p <0.05).3.Logistic regression analysis showed that when the optimal critical value of serum TSH was 3.01mIU/L,the sensitivity of TSH in diagnosing malignant nodules was80.32%,the specificity was 51.13%,and the AUC under ROC curve was 0.722.The optimal critical value of serum Tg is 60.21ng/ml,the sensitivity of Tg in diagnosing thyroid malignant nodules is 42.26%,the specificity is 43.29%,and the AUC under ROC curve is 0.611.The AUC value under ROC curve by serum TSH combined with Tg in the diagnosis of thyroid cancer was 0.781.4.With the increase of TSH,the incidence of thyroid malignant nodules also increases.Conclusion:1.The TSH level in serum is related to the nature of thyroid nodules.The average TSH level of malignant nodules is higher than that of benign nodules,which can be used as a good indicator to predict the nature of nodules.Within the normal range,with the increase of serum TSH,the more likely thyroid nodules are to become malignant.2.The Tg in serum can be used as a potential predictor of thyroid cancer.The average Tg level of malignant nodules is higher than that of benign nodules,but the accuracy is relatively low and there are limitations.It is not recommended.3.TSH combined with Tg in serum has a better accuracy and sensitivity in predicting thyroid cancer,and has a better effect in diagnosing the nature of nodules before operation.
