Objective:The purpose of this study is to explore the clinical characteristics,pathological types and risk factors of colorectal polyps between young and middle-aged patients and the elderly patients,so as to scientifically guide medical staff to provide personalized health education for different age groups,so as to reduce the incidence of colorectal polyps and the incidence of colorectal cancer.Method:This study was conducted from January 2019 to October 2019.With convenient sampling method,277 patients with colorectal polyps were selected as subjects,who were hospitalized in the Department of Gastroenterology of a first-class hospital in Jinan City,Shandong Province,and completed colonoscopy.The general data of patients were collected,including age,gender,nature of work,disease history,family history,medication history,lifestyle,eating habits,etc.the patients with polylps were consulted the colonoscopy report to collect the size,quantity,pathological type and location of polyps.A questionnaire survey was conducted by using the somatization self rating scale(SSS).According to the age,the patients were divided into young and middle-aged group and old-age group.According to whether the patients had polyps,the young and middle-aged group were divided into polyp group and polyp group.The old-age group was divided into polyp group and polyp group.Single factor analysis and multi factor analysis were used to explore the risk factors of colorectal polyps in the young and middle-aged groups and the elderly groups.Spss21.0 statistical analysis was used after data sorting,P<0.05 with statistical significance.Result:1.There was no significant difference between the two groups(P>0.05).2.The comparison of polyp location,pathological type,polyp size and polyp quantity between the young and middle-aged group and the old group showed that the proportion of polyps in the left and right half of the colon in the elderly group was higher than that in the young and middle-aged group(P<0.05);the proportion of polyp in rectum between the two groups was not statistically significant(P>0.05);the proportion of adenoma in the old group was higher than that in the young group(x2=17.618,P=0.000);the difference of polyp size between the old group and the young group was statistically significant(x2=6.687,P=0.035);the proportion of polyp in the old group was higher than that in the young group(x2=9.081,P-0.003).3.The results of univariate analysis of polyp formation in young and middle-aged group are different from those in old group.The results of single factor analysis of polyp formation in young and middle-aged group showed that:smoking history(x2=4.824,P=0.028),drinking history(x2=6.468,P=0.011),infrequent physical exercise(x2=10.060,P=0.002),irregular meals(x2=18.883,P=0.000),high frequency of eating animals'viscera(?1 time/week)(x2=6.254,P=0.01'2),high frequency of eating pickled products(?1 time/week)(x2=5.34 0,P=0.021),high frequency of eating fried food(?1 time/week)(x2=14.213,P=0.000),high frequency of eating spicy food(?1 time/week)(x2=14.335,P=0.000),insufficient fruit intake(<200g/day)(x2=8.121,P=0.017),history of malignant tumor in immediate family(x2=14.636,P=0.000),SSS? 30 points(x2=15.041,P=0.000)were the middle-aged and young group Risk factors for meat formation.Long term administration of antacids and gastric mucosal protective drugs(x2=11.537,P=0.001)were the protective factors for the formation of polyps in young and middle-aged people.The single factor analysis of polyp formation in the elderly group showed that BMI was more than 24(x2=12.174,P=0.000),diabetes history(x2=10.691,P=0.001),long-term constipation history(x2=8.810,P=0.003)were the risk factors of polyp formation in the elderly group.4.The results of multivariate analysis of polyp formation were different between the young and the old group.The results of multi factor analysis of young and middle-aged group show that Cancer(OR=0.140,95%CI:0.041-0.473,P--0.002),infrequent physical exercise(OR=3.087,95%CI:1.261-7.559,P--0.014),irregular meals(OR=4.380,95%CI:1.368-14.022,P=0.013),frequent fried foods(OR=3.452,95%CI:1.384-8.608,P=0.008),SSS? 30(OR=3.514,95%CI:1.324-9.326,P=0.012)were independent risk factors for polyps.Long term administration of anti acid drugs OR=8.969,95%CI:1.628-49.427,P=0.012)can reduce the occurrence of polyps,which is the protective factor of polyp formation.The results of multivariate analysis showed that BMI was more than 24(OR?4.736,95%CI:1.764-12.713,P=0.002),long-term constipation(OR=0.162,95%CI:0.041-0.644,P=0.010)and diabetes history(OR=0.137,95%CI:0.043-0.436,P=0.001)were independent risk factors of polyp formation in the elderly group.Conclusion:1.There was no difference in symptoms and signs between the young group and the old group.2.The proportion of polyps in the left and right half of the colon in the elderly is higher than that in the young and middle-aged;the proportion of adenomas in the polyps of the elderly is higher than that in the young and middle-aged;the proportion of polyps in the elderly is higher than that in the young and middle-aged.3.The immediate family members have malignant tumors,do not often participate in physical exercise,have irregular meals,and eat fried food times,the number of polyps and SSS?30 were the risk factors of polyp formation in young and middle-aged people.4.BMI over 24,long-term constipation and diabetes history are the risk factors of polyp formation in the elderly group.5.The risk factors of polyp formation are different between the young and middle-aged and the old.
