| Objective:To observe the clinical efficacy and safety of scallion white Tongyang paste in preventing heart failure in patients with acute myocardial infarction.Methods:90 patients who met the ami diagnosis were selected from the inpatient department of cardiovascular medicine in Wuhan first hospital from July 2016 to June 2018.90 patients were numbered according to admission order and divided into control group and treatment group according to random number table method.In the control group,45 cases were given western medicine routine treatment,the control group was given western medicine routine treatment plus scallion white Tongyang paste applied to bilateral Zusanli,bilateral Neiguan,bilateral Xinshu,Shenque 7 acupoints as adjuvant treatment,the two groups were treated for 28 days.To determine the score of TCM syndromes and calculate the rate of improvement of TCM syndromes before and after treatment;to assess cardiac function and calculate the effective rate of improving clinical symptoms of HF;to measure and record the lvesv?lvedv and lvef,serum nt-probnp of patients by echocardiography and compare;To evaluate the patients' mental health and physical health according to the sf-36 questionnaire,calculate and compare the total quality of life score;observe the occurrence of the main adverse cardiac events during the course of treatment and count the proportion;monitor the patients' liver and kidney function,electrolyte,blood sugar,blood lipid and so on,lvef,assess their safety.Data obtained from this experiment are processed using statistical software spss 23.0.The measurement data were expressed by mean ± standard deviation(`x±s),the comparison between groups was analyzed by t test,and the counting data between groups by chi-square test,with p x±s 0.05 as statistically significant.Result :1.In improving the symptoms and effective rate of TCM,the syndrome score of the control group and the treatment group decreased significantly,and the total effective rate of the treatment group was significantly lower than that of the control group after the end of the course of treatment(p<0.05).2.To improve the efficiency of heart failure,the total effective rate was 73.3% in the control group and 86.7% in the treatment group.total effective rate in the treatment group was significantly higher than that in the control group,and the difference was statistically significant(p<0.05).3.In terms of cardiac function,the lvef?lvesv?lvedv of the control group and the treatment group were significantly improved(p<0.05),and the improvement of the treatment group was significantly higher than that of the control group(p<0.05).4.In terms of serum nt-probnp,both groups decreased significantly(p<0.05),and the decrease in the treatment group was greater than that in the control group,and the difference was statistically significant(p<0.05).5.In terms of sf-36 quality of life score,the three indexes of mental health,physical health and total score of the two groups after treatment were significantly improved(p<0.05);the three indexes in the treatment group decreased significantly more than the control group,and there was a significant difference(p<0.05).6.The adverse events in the course of treatment,the incidence of cardiogenic shock and cardiac insufficiency in the treatment group was significantly lower than that in the control group(p<0.05),but there was no significant difference between the two groups(p>0.05).7.In terms of adverse reactions,two groups of three major routine,liver and kidney function,electrolyte and other safety indicators did not find special abnormalities,some patients had skin pruritus,gastrointestinal discomfort,untreated automatic disappearance.Conclusion:Scallion white tongyang paste can effectively improve the symptoms and clinical symptoms of ami patients,prevent heart failure,improve lvef?lvesv,reduce serum nt-probnp,improve the quality of life,reduce the incidence of some adverse cardiovascular events,improve the prognosis,safe and effective. |
