Arteriosclerosis obliterans of the lower extremities is the manifestation of atherosclerosis in the lower extremities.It is a systemic and progressive disease in the cardiovascular system.Its pathological process can appear for the first time in its 20s,and it appears obvious after decades Symptoms,the incidence in the population is about 12%to 14%.Clinical data show that high serum bilirubin levels are associated with a reduced incidence of vascular diseases,such as coronary artery disease,atherosclerotic disease,and carotid atherosclerosis are not related to conventional risk factors in the general population,high serum bile red the level of vegan will reduce the susceptibility to vascular diseases.The study on the correlation between high serum bilirubin levels and lower extremity arteriosclerosis obliterans is unclear.Objective:To explore the correlation between serum bilirubin and lower extremity arteriosclerosis obliterans,and to provide help and theoretical basis for the early diagnosis and clinical individualized diagnosis and treatment of lower extremity arteriosclerosis obliterans.Methods:A retrospective analysis of the clinical data of patients diagnosed with ASO in the lower extremities after screening by exclusion criteria from January 2013 to January 2020 in the Department of Vascular Surgery,Affiliated Hospital of Yangzhou University.The 104 patients who met the requirements were taken as the case group,including 65 males and 39 females,with an average age of(73.6±9.8)years.Diagnostic criteria:age>40 years;high-risk factors such as smoking,diabetes,hypertension,hyperlipidemia;clinical manifestations of arteriosclerosis obliterans of the lower extremities;arterial pulsation of the distal ischemic limb weakened or disappeared;ABI?0.9;color Imaging examinations such as Doppler ultrasound,CTA,MRA,and DSA showed lesions such as stenosis or occlusion of the corresponding artery.Meeting the above 4 diagnostic criteria can make a clinical diagnosis of ASO.According to Rutherford grading criteria,the cases were classified into Rutherford grade 0(R0,18 cases,17.3%),Rutherford grade 1(R1,5 cases,4.8%),Rutherford grade 2(R2,3 cases,2.9%),Rutherford 3(R3,19 cases,18.3%),Rutherford 4(R4,15 cases,14.4%),Rutherford 5(R5,29 cases,27.9%),Rutherford 6(R6,15 cases,14.4%);In addition,100 cases of medical examinations in our hospital at the same time were selected as the control group;all the research subjects underwent clinical data statistics and related laboratory examinations at the time of admission,and analyzed and analyzed the clinical data and examination results.Observe the difference in serum bilirubin levels between the two groups,and explore whether there is a correlation between serum bilirubin levels and the occurrence and severity of lower limb ASO.Results:1.The results of chi-square test,t test and rank sum test showed that there was no statistical significance between the two groups in terms of gender,age,history of hypertension,and history of diabetes(P>0.05).Compared with the control group,TBIL,DBIL,IBIL,NEU%,LYM%,and ALB of ASO patients with lower extremities were statistically significant(P<0.05)The levels of TBIL,DBIL,and IBIL in the case group were(10.44 ± 3.46)%,(3.25±1.36)%,(7.28 ± 2.83)%,which were lower than that of the control group(15.30 ± 3.28)%,(3.97 ± 1.40)%,(11.33±2.65)%.2.Multivariate Logistics regression analysis of ASO risk factors for lower extremities showed that:TBIL is an independent protective factor for lower extremity arteriosclerotic occlusive disease;LYM%is an independent risk factor for lower extremity arteriosclerotic occlusive disease.3.There is no obvious difference in TBIL,DBIL,IBIL levels between different Rutherford grades,and it cannot be considered that there is a linear correlation.Conclusions:Serum bilirubin level is significantly correlated with lower extremity ASO,and can be used as a reference index for early diagnosis of lower extremity ASO patients;TBIL is an independent protective factor for lower extremity ASO,and its level can guide the evaluation of lower extremity ASO.Therefore,serum bilirubin level monitoring is of great significance in the diagnosis of lower limb ASO.
