Objective:to observe the clinical efficacy of yiguan combined with acupuncture in the treatment of cervical tinnitus,comparative analysis of the difference in the efficacy of yiguan combined acupuncture and pure acupuncture in the treatment of cervical tinnitus,provide better ideas and methods for clinical diagnosis and treatment of cervical tinnitus in the future.Methods:72 patients with cervicogenic tinnitus who met the medical standards were selected from the department of acupuncture,department of otolaryngology and treatment area of affiliated hospital of changchun university of traditional Chinese medicine(time range: January to December 2019)and randomly assigned to treatment group and control group(there were 36 cases in each group,and 7 cases dropped out of the experiment,including 4 cases in the treatment group and 3 cases in the control group).Finally,32 cases in the treatment group and 33 cases in the control group were included.The treatment group adopted the method of yiguan and acupuncture.Control group adopts the method of simple acupuncture.Each yiguan operation lasts for about 6 to 8 minutes,and the acupuncture takes 30 minutes for each time.Yi can was treated on Tuesdays and fridays,acupuncture was performed once a day(2 days rest on weekends),and 5 days was a course of treatment.After 4 courses of treatment,the changes of tinnitus degree and grade before and after treatment in the treatment group and the control group,as well as the changes of blood flow velocity in vertebral artery and basilar artery were analyzed to evaluate the efficacy.Results:(1)After treatment,the mean flow velocity of the left vertebral artery in the treatment group was 28.47±4.46cm/s,the mean flow velocity of the right vertebral artery was 27.91±4.23cm/s,and the mean flow velocity of the basilar artery was 29.09±3.69cm/s.In the control group,the flow velocity of the left vertebral artery was 25.85±3.40,that of the right was 23.69±3.61cm/s,and that of the basilar artery was 26.70±4.36cm/s.After treatment,the blood flow velocity of vertebral artery and basilar artery in the two groups increased compared with that before treatment(P < 0.05),and the improvement was more obvious in the treatment group than in the control group(P < 0.05).(2)After treatment,the average score of tinnitus in the treatment group was 6.00±5.02 points,while that in the control group was 8.82±4.13 points.There were 19 grade 1 patients,4 grade 2 patients,1 grade 3 patient,1 grade 4 patient and 2 grade 5 patients in the tinnitus treatment group,and 11 grade 1 patients,15 grade 2 patients,1 grade 3 patient,3 grade 4 patients and 2 grade 5 patients in the control group.After treatment,the tinnitus score and tinnitus grade in both groups were lower than before treatment(P < 0.05),but the reduction in tinnitus score and tinnitus grade in the treatment group was more significant than that in the control group(P < 0.05).(3)After the treatment,the treatment group recovered 5 cases,showed obvious effect 16 cases,effective 8 cases and ineffective 3 cases,with the overall effective rate of 90.62%.In the control group,1 case was cured,11 cases showed obvious effect,16 cases were effective and 5 cases were ineffective,and the overall effective rate was 84.84%.The overall effective rate of cervical tinnitus in the treatment group was higher than that in the control group(P < 0.05).Conclusion:(1)Yiguancombined with acupuncture can better improve the average blood flow velocity of bilateral vertebral arteries and basilar arteries;Reduce the degree and grade of tinnitus.(2)Compared with simple acupuncture in the treatment of cervical tinnitus,yiguan combined with acupuncture has better efficacy,worthy of clinical application.
