ObjectiveTo explore the effect of nursing intervention based on cross theoretical model on the quality of life of patients with ankylosing spondylitis,and whether it can provide theoretical basis and practical reference for clinical nursing intervention.MethodsBy using the method of similar experimental study and convenient sampling,the patients with ankylosing spondylitis who have been confirmed in the rheumatology and Immunology Department and related departments of a top three hospital in Bozhou City and meet the inclusion and exclusion criteria,aged 15-40 years old,were selected as the study objects and divided into the intervention group and the control group,36 cases in each group.The control group was treated with routine nursing,and the intervention group was treated with cross theoretical model on the basis of routine nursing for 12 months.At the time of discharge,6 months after intervention and 12 months after intervention,the scores of behavior change stage,disease activity index(BASDAI),body function index(BASFI),VAS pain score and SF-36 quality of life were collected.According to IBM SPSS statistics 21.0,the database is established for statistical analysis.The measurement data are tested by means of mean ±standard deviation and t test.The count data are expressed by cases(%).The comparison between groups is tested by x2 test.The rank data are tested by rank sum of two independent samples.The repeated measurement data are analyzed by repeated measurement variance analysis.Test level a=0.05,that is,P<0.05,the difference was statistically significant.Results1.There was no significant difference between the two groups in general information(age,gender,marriage,education level,religious belief,work status,per capita monthly income of family,medical payment method,living condition,as family history,the earliest symptom time and complications)before intervention(P>0.05);2.There was no significant difference in baseline data before intervention between the two groups(P>0.05);3.Six months after the intervention and 12 months after the intervention,the number of people in the behavioral change stage of the two groups was statistically significant(P<0.05)compared with that before the intervention,and the comparison between the two groups was also statistically significant(P<0.05);compared with the scores of disease activity index(BASDAI),functional index(BASFI)and pain,the value of theintervention group decreased more significantly,and the difference was statistically significant(P<0.05))With the extension of the intervention time,the intervention effect is more obvious,and the intervention group is significantly better than the control group,the difference is statistically significant(P<0.05);in the intervention group,the scores of eight dimensions of quality of life are improved,the difference is statistically significant(P<0.05),the total scores of physiological function,vitality,spirit and physiology of the patients in the control group after intervention are compared with those before intervention The difference was not statistically significant(P>0.05).After the intervention,the scores of all dimensions(physiological function,physiological function,physical pain,overall health,vitality,social function,emotional function,mental health,physiological total score and psychological total score)in the intervention group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusions1.Cross theoretical model nursing intervention can promote the change of behavior stage of as patients;2.In terms of disease cognition and behavior,cross theoretical model intervention can strengthen the intention and determination of patients' health behavior management,and help patients to establish long-term and sustainable health behavior habits;3.In the aspect of relieving symptoms,nursing intervention based on cross theoretical model can delay the progress of as,relieve pain,enhance the function of body activity,reduce the rate of disability,and provide practical basis for clinical nurses;4.Nursing intervention based on cross theoretical model can improve the quality of life of as patients.
