Objective:1.To construct a scientific and standard rehabilitation training program for bladder function after spinal cord injury.2.Through the implementation of the established rehabilitation program for spinal cord injury and the comparison of clinical quantitative indicators, further improve the program to form a scientific, targeted and effective rehabilitation training program for bladder function of spinal cord injury. To provide scientific guidance standards for clinical nurses, provide convenient and effective self-exercise measures for patients, further control the occurrence of various complications of clinical urinary system, promote the recovery of bladder function and improve the quality of life.Methods:1.Evidence-based nursing was adopted as the basic guiding ideology in the research process.According to the high-level consensus measures recommended in the literature review, the preliminary rehabilitation training program of neurogenic bladder function caused by spinal cord injury was formed, which was demonstrated by expert meeting method.2.Using the method of similar experimental study,30 patients with spinal cord injury who were admitted to the rehabilitation department of a general hospital in Henan Province from January to December 2017 were selected as the control group, and 30 patients with spinal cord injury who met the exclusion criteria from January to December 2018 were selected as the experimental group. The experimental group included 30 patients with spinal cord injury who met the exclusion criteria. The neurogenic bladder early functional rehabilitation program was implemented.Clinical data were collected regularly for clinical efficacy analysis:knowledge of rehabilitation training, indicators of bladder function changes, urinary tract infection rate, changes in hospitalization days, patient satisfaction, etc.Comparability of patients'basic data was analyzed by sorting out patients' general data.Comparisons of differences between normal distribution measurements could be made by using two independent samples t test.Chi-square test was used to analyze the differences of incidence between the two research groups. The test results showed that P<0.05 indicated that there were differences in data between the two groups.Significant statistical significance.Results:1.By literature review and analysis, the primary rehabilitation training program of neurogenic bladder was established, which involved 8 aspects:evaluation and diagnosis of bladder basic function, clinical classification, rehabilitation training steps, basic drug support therapy, diagnosis and treatment of complications, health knowledge popularization, psychological health guidance, evaluation of bladder clinical function recovery.Capacity part.Subsequently, the expert meeting method was used to evaluate, sort out and adjust the contents, options, framework of the preliminary bladder rehabilitation training program, and to design a neurogenic bladder rehabilitation training plan for follow-up clinical research.2.At the end of the experiment, the changes of bladder function in the experimental group and the control group were compared to evaluate the effectiveness of the neurogenic bladder early functional rehabilitation training program in clinical work. The study found that the general data of the two groups were not statistically significant(P>0.05).After the intervention, the success rate of the intervention group was finally higher than that of the control group, and the difference was statistically significant(P<0.05).The intervention group was superior to the control group in terms of bladder improvement index, including increased bladder capacity, decreased residual urine volume, and increased frequency of spontaneous urination. The difference was statistically significant(P<0.05).The incidence of urinary tract infection in the intervention group was significantly lower than that in the control group, and the difference was statistically significant(P<0.05).The number of hospital stays in the intervention group was lower than that in the control group, and the difference was statistically significant(P<0.05).The evaluation of patient satisfaction in the intervention group was higher than that in the control group, and the difference was statistically significant(P<0.05).The patients in the intervention group had better knowledge and proficiency in the health knowledge of each dimension than the control group, and the difference was statistically significant(P<0.05).Conclusions:1.The clinical practice of SCI neurogenic bladder early rehabilitation training plan constructed in this study has achieved remarkable results, which conforms to the basic purpose of bladder function rehabilitation training program design.2.Neurogenic bladder management after spinal cord injury can be widely applied in clinic, and its effect on improving bladder function is remarkable.
