The Clinical Study Of CKD1-3 Stage In IgA Nephropathy Based On The Theory Of "Shaoyang Main Axis"

Background:IgA nephropathy is the most common primary glomerulonephritis,which is characterized by IgA deposition in the mesangial region of the glomerulonephritis.IgA nephropathy is currently lacking in specific treatment measures,Chinese medicine has certain advantages.At present,most of the domestic TCM treatment of IgA nephropathy syndrome differentiation methods are eight syndrome differentiation,commonly used type of treatment,stage treatment and other dialectical thinking,clinical efficacy is uncertain.According to many years of clinical experience in the treatment of nephropathy,Professor Ma Jipei,combined with the clinical manifestations of IgA nephropathy and its etiology and pathogenesis,and the theory of"Shaoyang main pivot" and "Shaoyang main half table and half li" in the Theory of Typhoid fever,think that Shaoyang cardinal disadvantage is the key to its pathogenesis.From shaoyang theory treatmen——" reconciliation shaoyang "french traditional chinese and other clinical medicine,the theory of typhoid fever has established the system of syndrome differentiation of the six meridians,which has been effectively guiding the clinical practice of doctors in the past dynasties for a long time,and has had an important influence.The foothold of the "reconciliation of Shaoyang" method is to adjust the Qi machine,balance Yin and Yang,from the overall point of view,so that good spirit is restored,evil spirit is gone.This study will evaluate the efficacy and safety of treating IgA nephropathy(stage CKD1-3)from Shaoyang,and explore its possible mechanism to provide new ideas and methods for treating IgA nephropathy with syndrome differentiation.Purpose of the study:To evaluate the efficacy of Shaoyang on the treatment of IgA nephropathy(CKD1-3),to analyze its efficacy from a clinical point of view,and to explore its possible mechanism of action.At the same time,the liver and kidney function and adverse reactions were observed to evaluate the safety of Shaoyang prescription.Methods of study:Patients with IgA nephropathy(CKD1-3)who met the inclusion criteria were randomly divided into the trial group and the control group according to the random number table.The control group was treated with irbesartan tablets(0.15g/tablet,l tablet/day)at the same time with irbesartan tablets(200ml each time,one in the morning and evening)for 12 weeks.Results:included 60 cases of effective analysis,including 30 in the trial group and 30 in the control group.There was no significant difference in the course,age,sex and other general data between the two groups,and there was no significant difference in the baseline index between the two groups.(1)After 3 months of treatment,the total effective rate of the trial group was 80%,which was significantly better than that of the control group by 33.33%(P<0.01),suggesting that the clinical remission rate of Shaoyang syndrome(CKD1-3 stage)of IgA nephropathy was significantly higher than that of the control group.(2)After 3 months of treatment,the two groups were compared with each other.The experimental group was more effective in reducing proteinuria,hematuria,raising albumin and improving TCM symptoms than the control group(P<0.05).Serum creatinine,serum urea nitrogen,serum uric acid,blood cholesterol,triglyceride and low-density lipoprotein were reduced in the test group but were not statistically significant(P>0.05)(3)After 3 months of treatment,compared with the baseline,the experimental histone urine and hematuria decreased significantly,and the difference was statistically significant(P<0.01).(4)After 3 months of treatment,compared with the baseline,the albumin le-vel in the test group was significantly increased,and the difference was statistically significant(P<0.01).(5)The serum creatinine,blood urea nitrogen,blood uric acid,blood lipids(including serum cholesterol,triglycerides,low-density lipoprotein)in the test group showed a decrease in blood creatinine,blood urea nitrogen,blood uric acid and blood lipids(including serum cholesterol,triglycerides,low-density lipoprotein)after 3 months of treatment,but there was no statistical difference(P>0.05).(6)After 3 months of treatment,the total effective rate of improvement of TCM syndrome in the experimental group was 90%,among which the improvement of single TCM syndrome was better than that of the control group in terms of bitter mouth,dry pharynx,dizziness,chest and flank,cold and heat,upset,fatigue and sore lumbar spine(p<0.05).(7)During the course of treatment,there were no significant adverse reactions in both groups.Conclusion:(1)According to the theory of "Shaoyang main pivot ",the treatment of patients with CKD1-3 stage of IgA nephropathy from Shaoyang has certain effect,which can improve the symptoms of traditional Chinese medicine,reduce the level of urine protein and hematuria,and increase the level of serum albumin.(2)Based on the theory of "Shaoyang main pivot ",it is safe to treat CKD1-3 from Shaoyang.
