Objectives:To observe the improvement of symptoms of Biling prescription in patients with diabetic patients with acute gout and the intervention of GLUT9 and MMP3 in serum of model rats.To explore the effect of Biling prescription on blood glucose,uric acid,blood sedimentation rate and other related indicators and its mechanism,and to evaluate the efficacy through the research results.Methods:1.Clinical observation A total of 60 diabetic patients with acute gout who met the inclusion criteria were randomly divided into a control group and a treatment group,each with 30 patients.The control group was given indomethacin tablets and pioglitazone tablets orally,while the treatment group was treated with Biling prescription on the basis of the control group for 2 weeks.Are calculated respectively in the two groups before and after treatment of TCM syndrome integral,were observed before and after treatment,clinical symptoms and blood glucose,blood uric acid(UA),blood sedimentation(ESR),hypersensitive creactive protein(hs-CRP)and homocysteine(Hcy),and other indicators of change.2.Experimental research Forty healthy male SD rats were selected,and 9 of them were randomly selected as the normal group after 1 week of adaptive feeding.The remaining rats were selected as the experimental group to receive streptozotocin 40mg/kg intraperitoneal injection to induce the diabetes model,with blood glucose ?16.7mmol/l as the model standard.After successful diabetes modeling,0.2ml of sodium uric acid solution was injected into the joint cavity of the experimental group.The contralateral swelling of the joint capsule was used as the injection standard to induce the gout model,and they were randomly divided into the model group,the control group and the treatment group.After the establishment of model,indomethacin and pioglitazone were given to the control group.On the basis of the control group,the treatment group was given with Biling prescription,the normal group and the model group were given the same amount of normal saline.After 2 weeks of continuous gavage,blood from the abdominal aorta was collected to determine the levels of blood glucose(PG),blood uric acid(UA),glucose transporter9(GLUT9)and matrix metalloproteinase3(MMP3).During the experiment,the normal group was fed normal diet and the experimental group was fed high-fat diet.Results:1.Clinical observation Clinical observation showed that compared with before treatment,FPG,2h PG,UA,hscrp,ESR and Hcy in the control group and the treatment group were significantly decreased(P<0.01),and the degree of decline in the treatment group was better than that in the control group(P<0.01 or P<0.05).VAS scores of the two groups were significantly improved after treatment compared with those before treatment(P<0.01),and the analgesic effect of the treatment group was better than that of the control group(P<0.01).After treatment,the scores of TCM syndromes in the two groups were significantly lower than before(P < 0.01),and the improvement in the treatment group was significantly higher than that in the control group(P<0.05).After 2weeks of treatment,the total effective rates of the control group and the treatment group were 73% and 89%,respectively,and the efficacy of the treatment group was better than that of the control group(P<0.01).2.Experimental research Experimental observation showed that after modeling,the body weight of rats in the model group decreased significantly compared with that in the normal group(P<0.01),and the body weight of rats in the experimental groups increased significantly compared with that in the model group(P<0.01),and the therapeutic effect of rats in the treatment group was significantly better than that in the control group(P<0.01).After treatment,the gait of rats in the four groups was significantly different(P<0.01),and the improvement in the treatment group was significantly better than that in the control group(P<0.05).Compared with the normal group,PG,UA,GLUT9 and MMP3 of rats in the model group all increased significantly(P<0.01),while the above indexes in the treatment group all decreased significantly(P<0.01),and the curative effect was more obvious than that in the control group(P< 0.01 or P<0.05).Conclusions:Biling prescription can significantly improve blood glucose and uric acid in patients with diabetes and acute gout,promote the excretion of uric acid,reduce the expression of ESR,hs-crp,Hcy and other related indicators,reduce the clinical symptoms of patients and improve the quality of life.Biling prescription can significantly down-regulated the expression levels of GLUT9 and MMP3 in model rats,improved the gait of rats and inhibited the inflammatory response.Confirm this prescription has obvious hypoglycemic,uric acid,anti-inflammatory analgesic effect.
