Objective:The purpose of this study was to observe the clinical efficacy(including the recovery of clinical symptoms and signs and improvement of quality of life)and recurrence of traditional Chinese medicine treatment of malignant intestinal obstruction,and analyze whether Chinese medicine external treatment can promote the recovery of gastrointestinal function in patients with malignant intestinal obstruction,And analysis of typical cases,in order to provide clinical evidence for the application of traditional Chinese medicine treatment in malignant intestinal obstruction.Method:In this study,through retrospective analysis,we collected hospitalizations from the Department of Traditional Chinese Medicine and Western Medicine of the First Affiliated Hospital of Anyi Medical University from October 2017 to December 2019,and had previously received external treatment of traditional Chinese medicine(including acupuncture,acupoint injection,enema of traditional Chinese medicine,58cases of malignant intestinal obstruction were treated with Chinese medicine external application,moxibustion or Chinese herbal ointment).Observe its clinical efficacy,symptom changes,signs changes,quality of life,relapsewithin 3 months and other indicators,statistically record symptom improvement and water fasting time,and discuss and analyze typical cases among them.Result:1.Clinical efficacy:all 58 cases,markedly effective in 27 cases(46.55%),effective in 18 cases(31.04%),ineffective in 13 cases(22.41%),the total effective rate was 77.59%.The shortest time for 45 patients to reach the"significant effect/effective"efficacy standard was 2d,and the longest was 12d.In 8 cases of complete intestinal obstruction,the total effective rate was 37.50%,in 50 cases of incomplete intestinal obstruction,the total effective rate was 84.00%(?~2=8.575,P=0.01<0.05),the difference was statistically significant.In 16 cases of Dynamic intestinal obstruction,the total effective rate was 87.50%,and in 42 cases of Mechanical intestinal obstruction,the total effective rate was 73.81%(?~2=1.249,P=0.318>0.05),the difference was not statistically significant.2.Changes in clinical symptoms:The number of patients who had improved symptoms such as exhaust,defecation,abdominal pain,bloating,and vomiting on the third and seventh days of treatment was significantly increased comparedwith before treatment(P<0.01).Anal exhaust recovery time(d)(?)±s is 4.28±1.93,anal defecation recovery time(d)(?)±s is 4.10±1.23,abdominal pain relief time(d)(?)±s is 3.85±1.22,and abdominal distension relief time(d)(?)±s It was 5.06±0.86,vomiting disappearance time(d)(?)±s was 3.11±1.20,and fasting time(d)(?)±s was 7.19±3.86.3.Changes in clinical signs:The number of patients recovering from abnormalsigns such as abdominal tenderness and hyperactive bowel sounds on the third and seventh days of treatment increased compared with that before treatment,P<0.01,which was statistically different.4.In terms of quality of life:KPS score (?)±s before treatment was 48.45±9.88,KPS score (?)±s after treatment was 67.24±22.07(Z=-5.927,P=0.000<0.05).Comparing the KPS scores before and after treatment,the results showed a significant efficiency of44.83%,an effective rate of 36.21%,a stability rate of 13.79%,and a decrease rate of5.17%.The total improvement rate(significant efficiency+effective rate)was 81.04%,and the total stabilization rate(significant efficiency+Effective rate+stability rate)is94.83%.5.Recurrence within 3 months:Of the 45 patients who met the evaluation criteria of“significant effect/effectiveness”,16 patients had intestinal obstruction againwithin 90days,and the incidence rate was 35.56%.The obstruction was effectively relieved,and MBO treatment was not effective in the remaining 5 patients.Conclusion:1.Malignant intestinal obstruction can achieve better clinical efficacy by using external treatment of traditional Chinese medicine.2.External treatment of traditional Chinese medicine can promote the recovery of exhaustion and defecation,effectively relieve the symptoms of abdominal pain,bloating and vomiting,shorten the time of fasting water,and help the recovery of signs for further follow-up treatment.3.External treatment of traditional Chinese medicine can improve the quality of life of patients with malignant intestinal obstruction.4.Traditional Chinese medicine treatment for malignant intestinal obstruction hasa lower clinical recurrence rate and has a higher clinical promotion value.
