Objective:To explore the prognostic value of neutrphil-to-lymphocyte ratio(NL R)in patients with advanced gastric cancer.Methods:From Januery 2010 to Decom ber 2017,clinical data about 420 patient diagnosed with advance gastric cancer w ho cannot to be treated with radical surgery.The first-line chemotherapy was the pl atinum-based chemotherapy with two drugs and the fluorouracil chemotherapy。NL R were calculate from the last blood routin before the first chemetherapy.Taking t hemedan value 2.43 as the cut-off,patient were divide into low NLR group(NLR<2.43,n=208)and hiegh NLR group(NLR≥2.43,n=212).The Progression-free sur vival(PFS)and overall survival(OS)of patients in two groups are compared.NL R were calculated from the last blood routine at 8±2 weeks after chemotherapy.T aking the median value 2.0 as the cutoff,patients were divided into low NLR gro up(NLR < 2 n=210)and high NLR group(NRL≥2.0,n=21O).The PFS and OS of patient in two group are compared again.Results:the mPFS and mOS of lo w NLR groups were higher than the hiegh NLR group,and the differences betwee n two group are statistically significant(6.23 months Vs 5.43 months,P <0.001、 17.46 months VS 10.90 months,P<0.001);After chemotherapy,the median PFS and OS of the low NLR group were also higher than the high NLR group,and th e differences between two group are statistically significant(6.60 months vs.5.13 months,P<0.001).16.8 months VS 10.90 months,P<0.001).COX Univariete analyis show that NLR,before chemotherapy,is a independont pregnostic factor of PFS a nd OS(P<0.001).After chemotherapy,NLR was also an independent prognostic fa ctor of PFS and OS(P< 0.001).Conclusion:NLR,before and after chemotherapy,is value in the pregnosis of patient with advancd gastric cancer who loses radical s urgical treatment and receives chemotherapy. |
