Objective: To explore the effect of integrated management model of specific diseases on heart failure in PD(peritoneal dialysis,PD)patients.Methods: This study was a self-controlled case study.50 patients who were selected for continuous ambulatory peritoneal dialysis for more than 6 months in the peritoneal dialysis center of the Fifth Affiliated Hospital of Xinjiang Medical University from January 2015 to December 2018,the risk factors and indicators of heart failure in peritoneal dialysis patients identified by retrospective analysis,implementation of integrated management of specific diseases for research subjects,repeated assessment,monitoring and improvement of obvervational index,through repeated cycle of quality improvement,to evaluate regularly the changes of heart failure related indicators in PD patients after integrated management of specific diseases compared with those before management.Results: After 3 and 6 months of integrated management of specific diseases,the results were compared with those before management: PD patients' blood pressure,serum albumin,hemoglobin,blood calcium,serum phosphorus,blood potassium,PTH,triglycerides,cholesterol,uric acid,infection,pleural effusion and so on are improved before management,there were no significant decrease in weekly Kt/V,weekly total Ccr and weekly renal Ccr;Patients with NT-proBNP,cardiothoracic ratio,and LVEF were better than before management and tended to normal values,there were statistically significant differences in NT-proBNP and cardiothoracic ratio(P < 0.05),and no statistically significant differences in LVEF(P > 0.05).Conclusion: The integrated management model of specific diseases may help to improve the occurrence and development of heart failure in PD patients and it is speculated that it may reduce the incidence of cardiovascular events in PD patients.
