| Objective: to explore the changes of brain microstructure in patients with ischemic stroke treated by manipulator based on diffusion kurtosis imaging((DKI)).Methods:fifteen patients with ischemic stroke with wrist and hand dysfunction diagnosed in our hospital from December 2018 to December 2019 were collected,and the manipulator rehabilitation exercise was carried out for 4 weeks.The patients were scanned by routine head scan and diffusion sequence before and after rehabilitation,and processed with DKE and other software.The average kurtosis coefficient(MK),average diffusion coefficient(MD),axial kurtosis coefficient(K∥),radial kurtosis coefficient(K⊥)and partial anisotropy fraction(FA)were calculated and the corresponding images were generated.The parameters of brainstem,splinter of corpus callosum,bilateral internal capsule,bilateral hippocampus,bilateral thalamus and bilateral precentral gyrus cortex were measured.Paired t-test was used to analyze and compare the two groups of data.to determine whether there is a difference between the two groups of data and use Siemens workstation to track the nerve fiber bundle.Results:(1)compared with those before treatment,the MK of brainstem,splinter of corpus callosum,bilateral internal capsule,bilateral hippocampus,bilateral thalamus and bilateral temporal lobe cortex increased,MD decreased,K ∥ increased,K ⊥ increased,and the difference was statistically significant(P < 0.05).The FA values of brainstem,splenium of corpus callosum,bilateral internal capsule,ipsilateral hippocampus,bilateral thalamus and bilateral precentral gyrus cortex after treatment were significantly higher than those before treatment(P < 0.05).There was no significant difference in FA value of contralateral hippocampus.(2)the changes of nerve fiber bundle conduction pathway were observed directly before andafter treatment by nerve fiber bundle tracking technique.Conclusion: this study can quantitatively,intuitively and non-invasively observe the changes of brain microstructure of stroke patients rehabilitated by manipulators through diffusion kurtosis imaging sequence(DKI),and provide timely diagnosis and treatment information and timely adjustment plan for clinic. |
