A Clinical Study On The Treatment Of Osteomyelitis Of Lower Extremity With Ring Combined With One Arm External Fixator

Objective:To investigate the clinical efficacy of Ilizarov ring combined with Orthofix single external fixator technique in the treatment of osteomyelitis at the lower extremity periarticular end of the knee joint in the treatment of infectious bone defects.Methods:Data of 8 patients with osteomyelitis of lower extremity femur and tibia near articular bone end treated by the first affiliated hospital of xinjiang medical university from November 2012 to November 2019 were retrospectively analyzed.All patients were treated with Ilizarov ring +Orthofix one-arm external fixator.During postoperative follow-up,the knee joint function was evaluated by Hss function score,and the bone defect length and soft tissue defect area were compared and statistically analyzed.Results:All patients were followed up for 8-24 months(mean 10.8 months)after the operation,during which all patients had no other injuries or deaths.The average length of bone defect in all patients was 4.3-9.6cm(average 7.73cm),the bone migration time was 28-90 days(53.5 days),the shelf time was 10-40 months(average 15.6 months),and the bone healing index was 3l d/cm.There was no significant difference between knee imaging before surgery and after disassembly.The knee HSS score before surgery and after disassembly was further improved,the bone defect length was restored,the soft tissue defect area was significantly reduced,and the knee motion was not further lost,the difference was significant(P<0.05).Conclusion:Using Ilizarov circular + Orthofix single arm joint external fixator,treatment of osteomyelitis of extremities around the knee joint is analyzed to obtain the exact clinical curative effect,external fixation firmly not loose,under control of infection,bone defect length,defect of soft tissue healing,knee joint activity is not lost,hurt knee functional score significantly increased after the fight.
