Two-dimensional Vaginal Ultrasound Combined With Clinical History Establish Endometrial Cancer Risk Scoring System

Objective: To explore the establishment of a scoring system to assess the risk of endometrial cancer by two-dimensional vaginal ultrasound combined with clinical history Determine the endometrial benign and malignant lesions,and use the cutoff as a method for the preliminary diagnosis of endometrial cancer.Methods: A total of 198 ultrasound patients with uneven endometrial echo were collected.The ultrasound parameters and clinical history were recorded.The selected variables were determined based on clinical experience and related literatures.Differences and correlation analysis were performed.Variables were scored and pathological results were used as gold.Standard,draw receiver receiver characteristic(ROC)curve and calculate Yoden index,calculate the critical score of the best scoring system and evaluate the diagnostic efficiency of the scoring system.Results: The variables were analyzed for menopause,irregular vaginal bleeding,history of diabetes,endometrial thickness,intrauterine contents,borders,and Doppler blood flow.The results were statistically different(P<0.05).The above-mentioned variables were analyzed by single factor Logistic regression analysis,and the results were statistically different(P < 0.05).The above-mentioned variables were analyzed by multi-factor Logistic regression analysis,and whether the variables were menopausal,vaginal bleeding,endometrial thickness,border,and Doppler blood flow were statistically analyzed.(P<0.05);according to the OR value of the variable,combined with clinical experience and related literature,assign a score to each variable,calculate the total score one by one and divide the benign and malignant group statistical scores,the benign and malignant group scores have statistical differences(P<0.05));Establish a scoring system for assessing the risk of endometrial cancer via two-dimensional vaginal ultrasound combined with a clinical history,draw a ROC curve,and take the area under the ROC Curve(AUC)to a maximum of 0.879 and 95%confidence interval,CI)0.815-0.943,the cutoff is 5.5,and the sensitivity and specificity are 70.8% and 94.0%,respectively.Conclusion: Establishing endometrial malignant disease risk scoring system through two-dimensional vaginal ultrasound combined with clinical history is of certain value for clinical diagnosis of endometrial cancer.
