A Endometrial Receptivity Ultrasonography Scoring System As A Method For The Prognosis Of Frozen-thawed Embryo Implantation

Objective: The objective of present study was to draw up a score form about the endometrial thickness,endometrial morphology,endometrial myometrial echo intensity,endometrial peristaltic wave frequency,endometrial blood flow distribution,endometrial blood flow distribution Blood flow branch),endometrial spiral artery PI value,then clinicians could evaluate the endometrial receptivity.To evaluate the efficacy of the scoring system in evaluation of endometrial receptivity and to explore other factors affecting endometrial receptivity,to provide a guideline for clinical evaluation of endometrial receptivity and predict the success probability of transplantation before clinical FET transplantation.Method: We have selected 184 infertility patients treated with IVF-ET between October,2015 and September,2016 as subjects to study endometrial receptivity.We used the GE LOGIQ7 Color Doppler Ultrasonography to perform transvaginal ultrasonography on the HCG day and evaluated the following parameters in the re-established scoring system above:(1)The endometrial thickness(<7 mm = 0,7-9 mm = 2,9-14 mm = 3,?14mm=1);(2)endometrial morphology(type A= 3;B type = 2;C type = 0);(3)Endometrial peristaltic wave frequency Per minute(1.1-2.0=3??1.0 =2?2.1-3.0=1??3.0=0);(4)Uterine muscle echo intensity(uniform = 2,rough or uneven =1);(5)Endometrial blood flow(Type?= 1,type?=2,type?=3);(6)endometrial blood flow distribution(arterial blood flow branch)[type ?: endometrial area without blood flow(0)= 0,Endometrial area of blood flow,but very sparse(? 2)= 1,? type: endometrial blood flow is more sparse(3-4)= 2,?: endometrial blood flow more(7)Uterine spiral arterial PI value: PI ?1.20 = 2,PI = 1.21-1.60 = 1,PI ? 1.60 = 0.The total score of the above items is divided into three groups: Group A: Integral ? 12;Group B: Points 13-15,Group C: Integral 16-20.Results: There were obvious differences in the outcome of the three groups(P <0.05),and the higher the score,the better the pregnancy outcome;? 12 group and 16-20 group,13-15 group and16-20 group age were obviously different(P <0.05).There was no obvious correlation between the three groups of infertility,infertility type,transplantation experience and endometrialreceptivity(P> 0.05).For the endometrial morphology,endometrial blood flow distribution,endometrial arterial blood flow branch,There was obvious correlation of the three groups(P<0.05).There were obvious correlation in the thickness of the three groups(P <0.05)between the?12 group and the 16-20 group.There was no obvious difference in intensity,endometrial peristaltic wave frequency and uterine spiral artery PI value(P> 0.05).Conclusion:1.Endometrial receptivity score table can be a better evaluation of endometrial receptivity,Can predict the pregnancy outcome of FET,can avoid poor tolerance of endometrial embryo transfer,and then transplanted when the endometrial receptivity is improved.2.endometrial receptivity in younger infertility patients is better.3.Endometrial receptivity is not affected by infertility years,infertility type,the number of transplanted,and may be associated with the number of previous successful mumber.4,endometrial thickness,morphology,endometrial and subendocardial perfusion is closely related to endometrial eceptivity.4.Endometrial receptivity is closely related to intimal thickness,endometrial morphology,intimal and subendocardial perfusion.
