Objective: to investigate the clinical and pathological features of primary IgA nephropathy with hyperuricemia.Methods: a total of 313 patients with IgA nephropathy diagnosed by renal biopsy from January 2009 to May 2019 were selected from the first affiliated hospital of xinjiang medical university.According to the serum uric acid level,the patients were divided into the normal group and the hyperuricemia group,and the clinical and pathological data of the two groups were compared.Results: among the 313 patients in this study,157 were in the hyperuricemia group.Systolic blood pressure [130(120,142)mmHg],serum creatinine [80(66.5,112.0)umol/L],blood cholesterol [4.87(4.08,6.10)umol/L],triglyceride [2.11(1.41,3.27)umol/L] were higher in the hyperuricemia group than in the normal uric acid group [120(110,130)mmHg,73(58,88)umol/L,4.51(3.81,5.43)umol/L,1.58(1.03,2.48)].On the contrary,eGFR[(75.06±27.67)ml/(min·1.73m2)] and HDL [1.01(0.90,1.30)umol/L] were lower than those in the normal uric acid group [(81.60±29.70)ml/(min·1.73m2)and 2.78(2.21,3.54)umol/L](P<0.05);The difference in the proportion of normal renal function and abnormal renal function between the two groups was statistically significant(P<0.05).Compared with the normal blood uric acid group,high uric acid hematic disease group of Lee's pathological grading ?-? proportion;Proportion of renal tubular atrophy/interstitial fibrosis(T1 + T2)in the Oxford pathological classification(P<0.05);The proportion,systolic blood pressure,serum triglyceride,serum high-density lipoprotein,serum creatinine,and serum uric acid levels were all higher in the moderate to severe lesion group(T1,T2)than in the mild lesion group(T0),while the eGFR levels were lower(P<0.05);SPearman correlation analysis showed that the serum uric acid level was positively correlated with the proportion of male patients,blood cholesterol,blood triglyceride,blood urea nitrogen andLee's pathological grade,and negatively correlated with eGFR(P<0.05);Multivariate Logistic regression analysis showed that age and hyperuricemia were risk factors for primary IgA nephropathy.Conclusions: 1.The prevalence of hyperuricemia in primary IgA nephropathy is high,and the majority of patients are male.2.The clinical manifestations and Lee's grade of IgA nephropathy patients with hyperuricemia were more severe.3.There were significant differences in renal tubular atrophy/interstitial fibrosis in the Oxford classification.4.Hyperuricemia and age are risk factors affecting IgAN,so it is of great clinical significance to prevent the occurrence of hyperuricemia.
