Clinical And Pathological Significance Of IgG Diposit In Glomeruli In Patients With IgA Nephropathy

Objective:(1)To investigate the clinical data,laboratory examination,pathological typing and drug comparison between IgA nephropathy with orwithout IgG deposition in patients.(2)To explore the clinical data,laboratory examination,pathological typing and drug comparison of IgA nephropathy patients with IgG deposition area.Methods:The data were collected from327 patients with IgA nephropathy diagnosed by renal biopsy in the First Affiliated Hospital of Nanjing Medical University from January 2015 to 2016.All renal biopsy samples were examined by light microscopy and immunofluorescence.IgA nephropathy patients were divided into IgA group(n245)and IgA-IgG group(n=82).All the patients were divided into two groups by Lee,Oxford and Hass.Results:(1)IgA nephropathy with IgG deposition group clinical 24 hours urine protein,serum ereatinine,uric acid,eomplement C4,hypertension than IgG negative group(P<0.05);eGFR was lower than that of IgG negative group(P0.003);pathology in IgG deposition group and IgG negative group renal tubular atrophy/interstitial fibrosis(T)had significant difference(P<0.05),proliferation of mesangial cells and other Lee type,Hass type,mesangial hjypercellularity?segmental glomerulosclerosis or adhesion?endocapillary cell hyperplasia in two groups had no statistically significant difference(P>0.05).In the drug,deposition group and negative group are no differenee.(2)patients with IgG mesangial area were younger than age,and had statistical significance for the history of hypertension.The eGFR?serum creatinine,urea nitrogen and uric acid in IgG with basement membrane deposit group were lower than those in the IgG system?And Lee and Hass and Oxford type in the patihology and there was no significant difference between the two groups(P>0.05).The drug,IgG basement membrane deposition group and basement membrane deposition group are no difference.Conclusion:The patients with IgA nephropathy with IgG deposition were younger,24 hours urine protein,serum creatinine,hypertension and other clinical data and pathology.The drug,according to ACEI/ARB,glucocorticoid and immune-suppressants to explore IgA nephropathy with IgG deposition group and negative group had no statistically significant difference in treatment.We should strengthen the understanding of IgA nephropathy with IgG deposition and delay the progress of IgA nephropathy.
