Objective To investigate the relationship between type 2 diabetes mellitus complicated with microvascular complications or OSAHS and serum total bile acid level.Methods(1)The newly diagnosed T2DM patients and healthy control group were selected to analyze the difference in total bile acid level between the two groups.The T2DM group was stratified according to serum TBA level,and its correlation with microvascular disease was analyzed.(2)T2DM patients were selected to analyze the difference in total bile acid level between the two groups who were complicated with OSAHS or not.The T2DM patients complicated with OSAHS were divided into subgroups according to the risk factors.The TBA level of each subgroup were compared,and the clinical significance of TBA in OSAHS patients was analyzed.Results(1)The serum TBA level of T2DM group was higher than that of NC group(P<0.05).Pearson correlation analysis showed that the serum TBA level in the T2DM group was positively correlated with FC-P and HOMA-?(C-P),and was negatively correlated with TC and LDL-C(P<0.05).Logistic regression analysis of risk factors for diabetic microangiopathy showed that TBA was its protective factor(OR=0.514,95%CI 0.114?0.826,P<0.05).(2)The proportion of men,BMI,HOMA-IR(C-P),serum TBA and GGT levels were significantly higher in the T2DM patients complicated with OSAHS than that of simple T2DM patients(P<0.05).In the subgroup analysis,the subgroup with higher BMI,higher HOMA-IR(C-P)and much more severe OSAHS had higher TBA levels(P<0.05).Binary logistic regression analysis indicated that TBA level was the influencing factor for the development of OSAHS in T2DM patients(OR=1.223,95%CI 1.078?1.389,P<0.05).Conclusion(1)Serum TBA level of patients with newly diagnosed T2DM is higher than that of healthy people,which may be a protective factor for microvascular disease in T2DM patients.(2)The TBA level of T2DM patients complicated with OSAHS is higher than that of simple T2DM patients,and it increases with the severity of OSAHS,which may be related to the oxidative stress,insulin resistance and obesity.BA may be involved in the pathophysiological process of OSAHS.
