| ObjectiveThis study aims to establish the evaluation index system of pneumonia associated with stroke,so as to manage the risks and detect the risk management,sense of control first,identifying high risk factors and steps.Thus effective measures can be taken to promote the management of hospital infection,and provide a systematic and scientific theory theoretical basis for scientific and systematic for the prevention of pneumonia associated with stroke.This study can provide as theoretical scientific basis for the management of hospital infection and clinical nursing work plans.Methods1.The literature of stroke-related pneumonia and risk assessment home and abroad was acquired by retrieving,consulting and extensively studying the literature on hospital's infection management,statistics and nursing management.The analysis of the literature provided a theoretical basis for the design of the study.2.In order to achieve the more comprehensive risk factors of stroke associated with pneumonia,open questionnaire and semi-structured interviews were conducted on 25 people from the Department of Infection Management and nurses.The questionnaire draft of the risk assessment impact was made and the basic framework of the risk assessment index system of pneumonia for patients with stroke was formed.3.Delphi method was used to improve and screen the pneumonia index system of the inpatients with stroke.The weight of the indexes at all levels was determined based on the two-round consultation of the sixteen experts and the analytic hierarchy process(AHP).The consistency test was also conducted.4.The questionnaire of the risk assessment impact of the stroke-related pneumonia was used as a testing tool to investigate 210 stroke specialists who met the standard.The data were sorted and analyzed by Excel and SPSS20.0 software.The reliability and validity of the questionnaire were tested by factor analysis,principal component analysis,correlation analysis and Cronbach's alpha coefficient.5.A total of 201 nurses who met the research criteria were surveyed with the questionnaire of the risk assessment impact of the stroke-related pneumonia which was tested well by reliability and validity.SPSS20.0 and AMOS24.0 softwares were used to and analyze the data,so as to construct the structural equation model(SEM)and verify the scientificalness of the index system.Results1.Based on the qualitative study six risk factors were analyzed and summarized.The basic framework of the nursing risk assessment index system of pneumonia with stroke was formed with six first-grade indexes including patients,iatrogenic factors,health-related performance,hospital staff,department management and ancillary system and 38 second grade indexes.2.The nursing risk assessment index system of pneumonia with stroke was established with 6 first-grade indexes,which includes such factors as patients,iatrogenic factors,health-related performance,hospital staff,department management and ancillary system and 33 second grade indexes.The weight of the indicators was calculated and the consistency was tested.3.The questionnaire of the impact of the nursing risk assessment of pneumonia with stroke was designed and later was conducted an empirical study.The reliability and validity of the index system were tested by the statistical analysis of every item of the questionnaire.The scientificalness and feasibility of the evaluation index system were verified by the structural equation model.Conclusions1.The influential factors of pneumonia risk in patients with stroke are summarized and analyzed with some qualitatively and quantitatively combined methods.The questionnaire which is compiled with good reliability and validity provides references for the construction of the index system.2.The index system helps managers identify the high-risk factors and high-risk steps,thus provides assessment basis for the effective prevention of nursing risks.It helps nurses take precautions,avoid medical risks and improve nursing management. |
