ObjectivesWith 14C-urea breath test(14C-UBT)as the gold standard,evaluate rapid urease test?RUT?,Hp stool antigen test?HpSA?,serum antibody test,three kinds of methods in detecting Helicobacter pylori?H.pylori?in the diagnosis of infection,provide important reference value for scientific rearch.Methods160 subjects were selected from October 2015 to October 2016 in the gastroscope room of Affiliated Hospital of Tai'an Medical College for gastroscopy.All patients comp-leted gastroscopy,while subjects in the day before gastroscopy performed 14C-breath test and stool antigen test,serum antibody.Results1.The positive rates of different methods were comparedRapid urease test positive rate was 56.88%?91/160?,the positive rate of stool antigen test was 54.38%?87/160?,the positive rate of serum antibody test was 60.63%?97/160?,three kinds of methods respectively and the positive rate of 14C-breath test?93/160?comp-ared to 58.13%,there were no significant differences?P>0.05?.2.The sensitivity and specificity of different methods were comparedAmong the 160 subjects,the highest sensitivity was rapid urease test?91.40%?,followed by fecal antigen test?81.72%?and serum antibody test?78.49%?.The specificity ranged from large to small for rapid urease test?91.04%?,fecal antigen test?83.58%?,and serum antibody test?73.13%?.The rate of coincidence was rapid urease test?91.25%?,stool antigen test?82.50%?and serum antibody test?76.25%?.3.Positive predictive value and negative predictive value of different methodsThe positive predictive value followed by rapid urease test?93.41%?,stool antigen test?87.36%?serum antibody test?80.22%?;negative predictive value,followed by rapid urease test?88.41%?,?76.71%?serum antibody test,stool antigen test?71.01%?.4.Consistency of different test methodsCompared with 14C-UBT,the rapid urease test showed a good agreement between Kappa and 0.75.The stool antigen test and the serum antibody test showed that the Kappa values were less than 0.75,compared with that of 14C-UBT,and there was a certain consistency between the values and the serum antibody tests.Conclusions1.14C-breath test as the gold standard for the diagnosis of Hp infection,Helicobacter pylori detection of non-invasive method of choice,can be used to evaluate the measurement of other methods of measurement;2.Rapid urease test diagnosis is better,consistent with 14C-UBT,can be used as a gastroscopy for the diagnosis of Hp infection;3.Fecal antigen detection for Helicobacter pylori infection screening,diagnosis and Hp radical treatment of follow-up,but also for large quantities of epidemiological survey;4.Serum antibody test as Hp screening test for the epidemiological investigation of Hp,but not as the first choice for the diagnosis of Hp infection,not recommended for Hp radical treatment after the review.
