| Objective:To analyse the characteristics of hydrogen proton magnetic resonance spectroscopy(~1H-MRS)of bilateral hippocampus in patient of diabetic encephalopathy by using MR scanner,and find the correlation between spectrum and clinical score,further assess its clinical value.Methods:Thirty cases with type 2 DE in Department of endocrinology of central hospital as the object of the present study and another 30 cases without type 2 DE in the same period of the object as control group.There were no statistical difference between the selected control group and the experimental group in the following three aspects:age,gender,level of education.Moreover all subjects were not associated with cognitive function of mental,nerve or related family history.For all the research object the Montreal Cognitive Assessment,conventional MRI and ~1H-MRS was carried on,and then selected bilateral hippocampal region as the region of interest.The metabolites such as N-Acetylaspartate,choline,and creatine were measured.The content of each metabolite at bilateral hippocam-pus of experimental group was correlated with MoCA scores.Results:1.Experimental group:The ratios of NAA/Cr in left and right sides of hippocampus were 1.096±0.71,1.096±1.43 respectively(P>0.05);Cho/Cr were 2.59±3.68,1.81±2.80(P>0.05).2.Control group:The ratios of NAA/Cr in left and right sides of hippocampus were3.90±7.42,5.05±16.21 respectively(P>0.05);Cho/Cr were 1.41±1.19,1.48±1.59(P>0.05).3.Experimental bilateral hippocampal NAA/Cr value is less than the control group(P<0.05),statistically significant difference;Cho/Cr value in left hippocampus is greater than the control group(P<0.05),difference is statistically significant;Cho/Cr value in right hippocampus is greater,and difference has no statistically significant(P>0.05).4.Experimental bilateral hippocampal NAA/Cr value is less than the control group(P<0.05),statistically significant difference;Cho/Cr value is greater than the control group(P<0.05),difference is statistically significantConclusion:1.~1H-MRS can display metabolism situation on bilateral hippocampal area in patients with type 2 DE.It may provide evidence for deeper analysis and reasoning of severe brain damage in patients with type 2 DE.2.The ratios of NAA/Cr and Cho/Cr were in correlation with clinical MoCA score.It can be used as early indicators for the diagnosis of brain injury in patients with type 2DE indicators. |
