Influencing Factors Analysis Of Hospitalization Costs In Children With Bronchopneumonia

Objective:(1)To explore the influential factors of hospitalization expenses in children with bronchial pneumonia, put forward reasonable measures to control the hospitalization expenses, in order to curb unreasonable or excessive medical treatment and save the limited health resources.(2) To understand the implementation status of single disease and clinical pathway, and raise reasonable suggestions and opinions about payment system for “single-diagnosed disease”.Methods:Firstly, we obtained relevant information by literature review,and designed the interview outline, then,we obtained the interview data by the semi-structure interview on three paediatricians, three hospital administrators and seventeen patients’ families;and then,we builded the cost control framework though analysing the interview data by using framework-based analysis and RQDA software;eventually, according to the information provided by the cost control framework,we analysed 408 cases of children with bronchial pneumonia by statistical methods, which were from three municipal hospitals of Shanxi Province.Results:By the method of semi-structured interviews,the study found: the hospitalization expenses of children with bronchial pneumonia had a larger fluctuation range. The influence factors of hospital costs can be divided into the controllable factors and uncontrollable factors.The core of cost control framework was the government system; the scientific hospital management is the core about the hospitals behaved well; patient is the supervisor. And eventually all subjects pointed to the hospital management.The statistical method analysis of 408 cases of children with bronchial pneumonia from Shanxi Province found:the major factors to influence the children’s hospitalization costs of bronchopneumonia were charges for medicine, comprehensive medical- treatment costs, diagnostic fees, medical consumables costs, the region, length of stay, age, the implementation of clinical pathway, the complications and gender. Compared to the first and the second, the costs of the third was higher about charges for medicine, diagnostic fees, medical consumables costs(P < 0.017); the percent was about 32.54% which was on the second hospital’s hospitalization days longer than the average, the third is about43.33%; compared the implementation of clinical pathway group and unimplementation,the charges for medicine and diagnostic fee were higher(P < 0.05), meanwhile, there was no statistical difference about the length of stay(P > 0.05).Conclusion:(1) The hospitalization expenses of children with bronchial pneumonia had a larger fluctuation range, which should be command between RMB 2699.757 to RMB 3344.449.(2) The major factors to influence the children’s hospitalization costs of bronchopneumonia were charges for medicine, comprehensive medical- treatment costs,,diagnostic fee, medical consumables costs, the region, length of stay, age. The controllable factors should be adjusted and controlled to control the unreasonable phenomenon of children’s bronchial hospitalization expenses.
