Part ? relationship between serum Cathepsin S,Cystatin C and properties of arterial plaqueObjective:To study the relation between serum levels of Cathepsin S(Cat S)and Cystatin C(Cys C)and the nature of arterial plaque in patients with large artery atherosclerotic cerebral infarction.Methods:Patients with large artery atherosclerotic cerebral infarction who were hospitalized in the Department of Neurology of the Third Affiliated Hospital of Southern Medical University and Kaifeng Central Hospital of Southern Medical University from October 2016 to December 2017 were included in the normal control group.According to cervical vascular ultrasound,it was divided into stable plaque group and unstable plaque group.According to the degree of vascular stenosis,it was divided into three subdivisions.All patients were enrolled on the first day of admission using the NIHSS scale for neurological deficit assessment;the second sky abdomen blood collection,enzyme-linked immunosorbent assay for serum Cat S,latex-enhanced immunoturbidimetric assay for serum Cys C,parallel neck Vascular ultrasonography and cranial MRA;After collecting the data,statistical analysis was performed using SPSS20.0 statistical analysis software.Results:This study included 168 patients with cerebral infarction,104 males and 64 females,with an average age of(55.6 ± 12.4)years,including 73 stable plaques,including mild stenosis,moderate stenosis,and severe stenosis.There were 15 places,37 places and 21 places respectively.In addition,95 unstable plaques were included,of which 58 cases,26 cases and 11 cases were mild stenosis,moderate stenosis and severe stenosis.There were 100 normal controls,55 males and 45 females with an average age of(54.0±10.3)years.Cat S(50.50±10.5 pg/mL vs.63.08 ± 20.80 pg/mL;p=0.03)and Cys C(0.81 ±0.10 mg/L vs.1.08±0.84 mg/L;p=0.007)was significantly elevated in patients with cerebral infarction.The unstable plaque group Cat S and Cys C were higher than the stable plaque group.The difference between the two groups was statistically significant(p=0.006).Cat S and Cys C levels were significantly different in the stenosis group,the moderate stenosis group,and the severe stenosis group(p=0.003).The more severe the stenosis,the more obvious the increase in Cat S and Cys C,and the degree of vascular stenosis was positively correlated with serum Cat S level.Conclusions:The worse the stability of carotid plaque in patients with large artery atherosclerotic cerebral infarction,the more severe the stenosis,the higher the serum Cat S and Cys C level,and it is positively correlated.Part ?:Serum Cathepsin S/Cystatin C and the prognosis of cerebral infarction and ischemic strokeObjective:To investigate the predictive effect of Cat S and Cys C on prognosis of ischemic stroke patients with large artery atherosclerotic cerebral infarction.Methods:Patients with large artery atherosclerotic cerebral infarction who were admitted to the Department of Neurology Department of Neurology of the Third Affiliated Hospital of Southern Medical University and Kaifeng Central Hospital of Southern Medical University from October 2016 to December 2017 were included.The healthy subjects were healthy controls.The patients in the disease group were tested by enzyme-linked immunosorbent assay for serum Cat S,latex-enhanced immunoturbidimetric assay for serum Cys C,and the general data and past history of the patients were recorded.The control group received blood samples from the medical examination center in our hospital for blood tests,CatS,Cys C and blood lipids,blood sugar,etc.,and record the general situation and past history.Multivariate logistic regression analysis of the degree of disability in the modified Rankin scale for 90 days after dischargeResults:Multivariate regression analysis showed that serum Cat S and Cys C levels were significantly associated with short-term prognosis in patients with large artery atherosclerotic cerebral infarction.The higher the Cat S and Cys C expression level,the worse the prognosis.Conclusions:Serum Cat S and Cys C were directly correlated with the occurrence of large artery atherosclerotic cerebral infarction,and also correlated to short-term mRS score,and had a predictive effect on short-term prognosis of cerebral infarction.
