Clinical Analysis Of 155 Cases Of Non-large-artery Atherosclerotic Cerebral Infarction

Objective To analyze the causes and risk factors,neurological deficit,imaging examinations,treatment and prognosis of the patients with non-large-artery atherosclerotic cerebral infarction.Methods 155 cases of consecutive patients with non-large-artery atherosclerotic cerebral infarction were included.They were divided into different groups based on the TOAST criteria and their clinical features were analyzed.Results We enrolled 155 patients(99 males and 56 females)in this study.The classification of the TOAST subtypes in this group were stroke of small-artery occlusion(97cases,62.6%),stroke of cardioembolism(26 cases,16.8%),stroke of undetermined cause(20 cases,12.9%),stroke of other determined cause(12 cases,7.7%).The risk factors in this group were hypertension(96 cases,61.9%),hyperhomocysteinemia(65 cases,41.9%),smoking(56 cases,36.1%),diabetes(51 cases,32.9%),hyperlipemia(49 cases,31.6%).The hypertension(74.2%,?2=18.106,P<0.01)and hyperlipemia(40.2%,?2=10.818,P<0.05)increased significantly in stroke of small-artery occlusion.The patients with stroke of small-artery occlusion had the lowest neurological deficit score(2.93±2.40)and the patients with stroke of cardioembolism had the highest neurological deficit score(7.58±4.74)(?2=21.797,P<0.05).The average ESRS score of stroke of cardioembolism was higher than stroke of other determined cause and stroke of undetermined cause respectively(F=4.134,P<0.05,P<0.05).The average ESRS score of stroke of small-artery occlusion was higher than stroke of other determined cause and stroke of undetermined cause respectively(F=4.134,P<0.05,P<0.05).Conclusion The stroke of small-artery occlusion had the largest percentage and the stroke of other determined cause had the smallest percentage in non-large-artery atherosclerotic cerebral infarctions.The stroke of cardioembolism appeared mostly in the middle-aged people and the old people and the stroke of other determined cause tended to occur in the young adults.The hypertension or hyperlipemia were more significant in stroke of small-artery occlusion.The patients with stroke of small-artery occlusion had the lightest neurological deficit and the patients with stroke of cardioembolism had the most severe neurological deficit.The causes of non-large-artery atherosclerotic cerebral infarction were complicated.Different types had obviously different clinical features.We should give individual treatment and evaluate prognosis clinically according to different causes.
