Objective:The study detected the expression levels of IL-22,IL-4 and IFN-y in serum of patients with chronic obstructive pulmonary disease(COPD)in remission stage.To investigate the expression levels and significance of various inflammatory factors in different grades of COPD.Method:From December 2016 to December 2017,67 patients with COPD in remission stage in the second ward of respiratory medicine of the second affiliated hospital of Kunming Medical University were selected as the experimental group,after lung function examination,according to the estimated value of forced expiratory volume in the diagnostic and grading criteria of GOLD 2017,the patients were divided into four groups:mild(15 cases),moderate(18 cases),severe(16 cases)and extremely severe(18 cases).The control group included 27 subjects who underwent physical examination center of the second affiliated hospital from December 2016 to December 2017,according to smoking status,the subjects were divided into control no smoking group(12 cases)and control smoking group(15 cases).The levels of IL-22 IL-4 and IFN-y were detected by enzyme-linked immunosorbent assay(ELISA).Meanwhile,lung function indicators(FEV1,FEV1%Pred,FEV1/FVC%and RV/TLC(%))were evaluated by lung function instrument.Results:1 There was no significant different in smoking index between the control group and COPD group(P=0.477).2 FEVi,FEVi%Pred,FEV1/FVC%and RV/TLC(%)of different groups(except control no smoking group and control smoking group)were significantly different in pairwise comparison(P<0.05).That is,the values of FEV1,FEV1%Pred and FEV1/FVC(%)in mild group,moderate group,severe group extremely severe group were all lower than those of the control no smoking group and the control smoking group with significant difference(P=0.000).Meanwhile,the values of FEV1,FEV1%Pred and FEVi/FVC(%)decreased with the increasing severity of COPD.The RV/TLC(%)values of mild group,moderate group,severe group and extremely severe group were all higher than those of the control non smoking group and the control smoking group,with significant difference(P=0.000).3 The levels of interleukin-22,interleukin-4 and interferon-y were not significant in the control no smoking group and the control smoking group(P>0.05).The level of interleukin-22 was significantly higher in the mild,moderate,severe and extremely severe groups than in the control no smoking group and the control smoking group(P<0.05).The levels of interleukin-4 and interferon-y in the mild group were not statistically significant compared with those in the control non-smoking group and the control smoking group(P>0.05),While the other pairwise comparisons were statistically significant(P<0.05).The expression levels of interleukin-22,interleukin-4 and interferon-y were gradually increased as the severity level of COPD remission patients increased.4 FEV1,FEV1%Pred and FEV1/FVC(%)were negatively correlated with IL-22,IL-4 and IFN-?,respectively(R=-0.473,R=-0.471,R=-0.481,R=-0.767,R=-0.890,R=-0.827,R=-0.708,R=-0.826,R=-0.770).RV/TLC(%)was positively with IL-22,IL-4 and IFN-y(R=0.463,R=0.789,R=0.711).The difference was statistically significant(P<0.01).Conclusion:1 IL-22,IL-4 and IFN-y were involved in the chronic inflammatory response in patients with COPD remission.2 The expression level of IL-22 increases with the increasing severity grade of COPD remission patients.3 The expression levels of IL-4 and IFN-y in the mild COPD remission group were not different from those in the control group.However,with the increasing the severity of COPD patients with remission,the expression levels of IL-4 and IFN-y were also gradually increased.The change trend to suggested that the monitoring of the expression levels of IL-4 and IFN-y in clinical practice could provide some basis for evaluation of the severity of COPD patients.4 IL-22,IL-4 and IFN-y can provide certain basis for the evaluation of disease progression and severity grading in the remission phase of COPD,and IL-22 can more accurately reflect the severity of patients in the remission phase of COPD.
