Comparative Study Of Microwave And Polyglycerol In The Treatment Of Thyroid Cystic Lesions

Objective:To investigate the efficacy of ultrasound-guided microwave ablation and percutaneous fine-needle lauromacrogol injection ablation in the treatment of thyroid cystic lesions..Methods:A total of 62 patients with cystic or cystic nodules were enrolled in our department from November 2016 to May 2018All patients included in the study were confirmed as benign lesions by preoperative puncture pathology.According to the patient's choice of treatment,the patients were divided into MWA group(n=36)and PLI group(n=26).After ultrasound-guided treatment,the intraoperative adverse reactions were observed,and follow-up was performed one by one at 3,6,and 12 months after surgery.The thyroid function was reviewed and the ablation was performed by ultrasound.The thyroid lesion volume reduction rate was compared for efficacy evaluation and patient satisfaction surveyResults:1.Intraoperative adverse reactions:MWA group,intraoperative pain in 4 cases,the total incidence was 11.1%;PLI group,intraoperative pain in 1 case,the overall incidence was 3.8%(P<0.05).2.Postoperative complications:All patients had no serious complications such as recurrent laryngeal nerve injury,hoarseness,tracheal injury and major bleeding.One case of pre-neck infection occurred in MWA group.3.Follow-up and efficacy:Ultrasound Adler semi-quantitative grading to assess ablation:3 months,6 months,12 months after surgery in MWA group:36 nodules were grade 0.3 months,6 months,12 after surgery in PLI group:24 nodules are grade 0,and 4 nodules are grade 1.Volume reduction rate of the two groups:at 3,6,and 12 months after operation,the volume reduction rate of thyroid cystic lesions in MWA group(48.74±24.89%,72.73±17.45%,89.84±4.14%)was significantly greater than that of PLI group(33.58±15.75).%,53.87±14.51%,67.12±13.88%)(P<0.05);postoperative thyroid function:the postoperative thyroid function indexes of both groups were within the normal range.Efficacy:The total effective ORR of the MWA group was 100%(P<0.05),while the PLI group(ORR)was 84.6%(P<0.05).Conclusion:Both MWA and PLI have the advantages of high safety,remarkable curative effect,small trauma,short operation time,high patient satisfaction,no damage to nail function,and are effective treatment methods for thyroid cystic lesions.There were two ways to treat thyroid cystic lesions,and their volume decreased with time.The volume reduction rate was the highest at 12 months.Compared with PLI,MWA is more effective than PLI,but the adverse reaction rate is higher than PLI.
