Association Between Interleukin-6,TNF-? Gene Polymorphism And Sarcopenia In Maintenance Hemodialysis Patients

Objective:To investigate the relationship between interleukin-6(IL-6),tumor necrosis factor-?(TNF-?)gene polymorphisms and susceptibility to sarcopenia in maintenance hemodialysis(MHD)patients.Methods:A total of 82 patients with non-hospital maintenance hemodialysis(MHD)who underwent regular dialysis and stable disease were examined by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)to detect IL-6 gene promoter region-174G/C,-572C/G and tumor necrosis factor-a(TNF-?)gene promoter region-308G/A,-863 C/A polymorphism,using bioelectrical impedance(BIA)to determine the patient's muscle mass,and the skeletal muscle mass index was calculated,the patient's grip strength and pace were measured,and relevant laboratory indicators were detected.The diagnosis of sarcopenia is based on the diagnostic criteria developed by the European Working Group on Sarcopeniain Older People(EWGSOP)in 2010.Statistical analysis was performed using SPSS 17.0 software.P<0.05 is statistically significant for the difference.Results:1.In 82 patients with MHD,the incidence of sarcopenia was 26.9%(22/82),including 38.2%(21/55)for males and 3.7%(1/27)for females.The incidence of men is significantly higher than that of women.2.There were no significant differences in age,duration of hemodialysi,albumin,serum phosphorus,25-hydroxyvitamin D3,TNF-?,skeletal muscle mass index,and pace between the two groups with or without sarcopenia,P>0.05.In addition,total protein,creatinine,blood calcium,triglycerides,cholesterol,and low density lipoprotein were similar between the two groups.However,the hsCRP and IL-6 in the sarcopenia group were significantly higher than those in the non-sarcopenia group(P<0.05).The TNF-? in the sarcopenia group was higher than that in the non-sarcopenia group,but the difference was not significant(P>0.05);the body mass index and grip strength of the sarcopenia group were significantly lower than those without the sarcopenia,and the difference was significant(P<0.05).3.IL-6-174G/C was predominantly GG in MHD patients and only 1 in GC;the genotype frequency of IL-6-572CC,GG,and GC were 68.2%,13.6%,and 18.2%in the sarcopenia group,and 43.3%,11.7%,and 45.0%in the non-sarcopenia group.The frequency distribution of this gene was not of statistical significance between the two groups(P=0.065>0.05).The genotype frequency of TNF-a-308GG and GA were 90.9%and 9.1%in the sarcopenia group and 90.0%and 10.0%in the non-sarcopenia group,without significant diffierences(P>0.05);The genotype frequency of TNF-a-863CC,AC were 40.9%,59.1%in the sarcopenia group,71.7%,28.3%in the non-sarcopenia group,and the frequency distribution of the gene was statistically significant in the two groups(P=0.01<0.05).Conclusions:1.The incidence of sarcopenia is high in patients with MHD,and the TNF-a-863C/A gene polymorphism is associated with the occurrence of sarcopenia in MHD patients,heterozygous AC is more likely to cause sarcopenia,and can be used as a test for susceptibility to sarcopenia in MHD patients.2.The-308G/A gene polymorphism is not associated with the onset of sarcopenina in MHD patients;3.The IL-6-174G/C and-572C/G gene polymorphisms were not associated with the onset of sarcopenina in MHD patients.
