Study On The Correlation Of TCM Syndromes With Fundus Photographs And Coagulation Parameters In Type 2 Diabetic Retinopathy

Objective:In this study,we collected the clinical data of coagulation parameters and fundus photographs in patients with type 2 diabetic retinopathy(T2DR)to explore the correlation between TCM syndrome types and research indicators,and to distinguish TCM syndrome differentiation and TCM prevention and treatment of diabetes.Retinopathy provides a theoretical and practical basisMethod:240 patients with T2DR who were admitted to the Department of Endocrinology,Gansu Traditional Chinese Medicine Hospital and Gansu Provincial Hospital of Traditional Chinese Medicine from January to December 2018 were collected.The names,gender,age and four medical records of the patients were collected.According to TCM syndrome type diagnostic criteria,syndrome differentiation:yin and jin deficiency,dry heat endogenous syndrome;qi and yin deficiency,collateral stagnation syndrome;liver and kidney deficiency,eye dysfunction syndrome;spleen loss health,water and moisture block Certificate;yin and yang deficiency,blood stasis syndrome 5 syndrome groups.The tested patients were tested for prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FBG),Platelet count(PLT),glycosylated hemoglobin(HbAlc),the fundus photographs were collected.Analyze the distribution of TCM syndromes and their correlation with research indicators.All observation data were collected and entered into the database,and multi-class logistic regression was used for statistical analysisResult:1.TCM syndrome distribution of 240 patients with T2DR:57 cases(23.75%)of dry and heat endogenous syndrome;54 cases of qi and yin deficiency and collateral stagnation syndrome(22.50%);liver and kidney deficiency,eyesight 49 cases of literacy certificates(20.42%);Spleen lost health,42 cases of water-dampness syndrome(accounting for 17.50%);yin and yang deficiency,blood stasis syndrome 38 cases(accounting for 15.83%)2.In 240 patients with T2DR,the PLT parameters from high to low were spleen loss and health,water and dampness syndrome,yin and jin deficiency,dry heat endogenous syndrome,qi and yin deficiency,collateral stagnation syndrome,liver and kidney deficiency,eye The collateral dystrophic syndrome,yin and yang deficiency,blood stasis syndrome,the difference was statistically significant(P<0.05).Multi-class Logistic regression analysis showed that PLT and qi and yin deficiency,collaterals and stagnation syndrome,spleen loss and health,water and dampness syndrome,yin and jin deficiency,dry heat endogenous syndrome were positively correlated,and yin and yang deficiency,blood stasis Condensation is negatively correlated3.In 240 patients with T2DR,the PT parameters from high to low were spleen loss and health,water and dampness syndrome,yin and jin deficiency,dry heat endogenous syndrome,qi and yin deficiency,collateral stagnation syndrome,liver and kidney deficiency,eye The dystrophic syndrome,yin and yang deficiency,blood stasis syndrome,the difference was statistically significant(P<0.05);FBG parameters from high to low were yin and yang deficiency,blood stasis syndrome,liver and kidney deficiency,eye Collateral dystrophic syndrome,Qi and Yin deficiency,collateral stagnation syndrome,yin and jin deficiency,dry heat endogenous syndrome,spleen loss and health,water and dampness syndrome,the difference was statistically significant(P<0.05).The results of multi-class logistic regression analysis showed that PT and qi and yin deficiency,collateral stagnation syndrome,spleen loss and health,water and dampness syndrome,yin and jin deficiency,dry heat endogenous syndrome were positively correlated.FBG was negatively correlated with qi and yin deficiency and collateral stagnation syndrome,and positively correlated with yin and yang deficiency and blood stasis syndrome4.In 240 patients with T2DR,the main TCM syndrome types in the fundus stage ? and ?were yin and jin deficiency,dry heat endogenous syndrome,spleen loss and health,water and dampness syndrome,qi and yin deficiency,collateral stagnation syndrome,and stage ? liver Renal deficiency and Mianluo deficiency syndrome were the main factors.In stage ?,yin and yang deficiency and blood stasis syndrome were the main factors,and there were differences between the groups(P<0.05).The results of multi-class logistic regression analysis were obtained.The fundus staging and qi and yin deficiency,collaterals and phlegm stasis syndrome,spleen loss and health,water and dampness syndrome,yin and jin deficiency,dry heat endogenous syndrome were negatively correlated,positively correlated with yin and yang deficiency,blood stasis syndrome5.Logistic regression analysis showed that 240 cases of T2DR patients with HbA1C were positively correlated with spleen loss and water and dampness syndrome,and negatively correlated with yin and jin deficiency and dry heat endogenous syndrome.Age was negatively correlated with qi and yin deficiency,collaterals and stagnation syndrome,spleen loss and health,and water-dampness block,and positive correlation with yin and yang deficiency and blood stasis syndrome.Conclusion:1.From the distribution of syndrome types,the TCM syndromes of T2DR patients are mainly distributed in the deficiency of yin and jin,the endogenous syndrome of dry heat;the deficiency of Qi and Yin,the collaterals of collaterals;the deficiency of liver and kidney,and the loss of collaterals.2.PLT and coagulation parameters are closely related to the development of T2DR.TCM"blood stasis syndrome" is an important cause of abnormal platelet count and coagulation parameters in T2DR patients.With the aggravation of TCM symptoms,PLT and PT gradually decreased,and FBG gradually increased.3.There is a correlation between TCM syndrome type and fundus staging in patients with T2DR.With the disease,the spleen loses health and transportation,the water and dampness block syndrome?Qi and Yin deficiency,collaterals and phlegm stasis syndrome?liver and kidney deficiency,Mianluo deficiency certificate?Yin and Yang In the process of development of blood deficiency and blood stasis syndrome,the fundus stage gradually increased with the aggravation of pathological manifestations.
