Objectives: 1.To explore the significance of early renal function monitoring in hypertensive disorder complicating pregnancy,and to provide scientific evidence for disease prevention and nursing intervention.2.To explore the nursing needs of patients with early-stage renal hypertension and to develop corresponding nursing strategies according to their nursing needs,and to provide corresponding nursing measures.Provides a nursing model for clinical care of patients with impaired renal function in patients with hypertensive disorder complicating pregnancy.Methods: This study is divided into two parts.Part I: Clinical observation of early renal injury in patients with hypertensive disorder complicating pregnancy.156 pregnant hypertensive patients in the maternity ward of a top three hospital in Zhenjiang City from August 1,2017 to April 1,2018,including 78 patients with mild pre-eclampsia and 78 patients with severe pre-eclampsia,with 40 participants Women with normal pregnancy were the control group.Monitoring renal function indicators creatinine,uric acid,urea nitrogen,urine protein,serum cystatin C,urine microalbumin,retinol binding protein.Analyze the condition of early renal damage,develop the nursing strategy of corresponding kidney injury,take effective nursing interventions,improve the condition of renal function damage,and reduce complications.Part II: Qualitative study of patients with early kidney injury during pregnancy.Twelve patients with early-stage renal hypertension were selected for face-to-face in-depth interviews to understand their health and nursing needs,and to develop nursing strategies based on their needs,problems and opinions,so as to provide a scientific basis for personalized nursing measures.Results: 1.Early renal function in patients with hypertensive disorder has different degrees of damage.Muscle liver,uric acid,urea nitrogen,urine protein,serum cystatin C,urinary microalbumin,retinol binding protein and other indicators reflecting renal function are significantly elevated.High-grade,severe pre-eclampsia is more severe in early stage of kidney injury than in patients with mild pre-eclampsia.Therefore,in the case of renal damage in hypertensive patients with pregnancy,appropriate nursing strategies should be developed,and effective nursing interventions should be taken to improve the renal damage in hypertensive patients during pregnancy.2.Patients with early kidney injury during pregnancy have a large need for care in terms of disease knowledge,access to knowledge,psychological care,dietary guidance,lifestyle,family,and medical staff.In clinical work,it is necessary to communicate with patients in a timely and comprehensive manner,to understand their accurate appeals,and to provide targeted care.Conclusions: 1.Early renal function in patients with hypertensive disorder has different degrees of damage.Muscle liver,uric acid,urea nitrogen,urine protein,serum cystatin C,urinary microalbumin,retinol binding protein and other indicators reflecting renal function are significantly elevated.High-grade,severe pre-eclampsia is more severe in early stage of kidney injury than in patients with mild pre-eclampsia.Therefore,in the case of renal damage in hypertensive patients with pregnancy,appropriate nursing strategies should be developed,and effective nursing interventions should be taken to improve the renal damage in hypertensive patients during pregnancy.2.The nursing needs of patients with early-stage hypertension and early kidney injury have the need for disease knowledge,family companionship,psychology,diet,healthy lifestyle,and sleep.Therefore,medical personnel need to provide nursing strategies from various aspects,improve nursing measures,and provide a scientific basis for nursing measures for patients with early-stage renal hypertension.
