Objective:Comparative outcome of total knee arthroplasty between three-dimensional printing patient-specific instrumentation and conventional instrumentation for osteoarthritis by meta-analysis.Methods: Computer search cochrane Library, embase, pubmed,CNKI,Wanfang Database. To collect the clinical randomized controlled trials of traditional knee arthroplasty and 3D printed individualized guide plate assisted knee arthroplasty for the treatment of osteoarthritis, and formulate the inclusion and exclusion criteria, evaluate the quality of the literature, extract and screen the data. The five items of postoperative mechanical force line of lower extremity(the angle of mechanical axis between femur and tibia)after knee joint function after, the time of operation and the amount of blood loss during operation were analyzed by using statistical software: review manager 5.3.Postoperative complications were analyzed by meta. Results: A total of 883 patients were selected and collected 12 articles of high quality clinical randomized control, including 441 cases with conventional total knee arthroplasty and 442 cases with three-dimensional printing assisted total knee arthroplasty.By meta-analysis,intraoperative blood loss(MD-167.23,95% CI[-305.04,-29.42] P = 0.02)suggested that there was less intraoperative blood loss in 3D instrumentation. The mechanical force line of lower extremity(MD-0.77,95% CI[-1.40,-0.13] P = 0.79)suggests that the 3D instrumentation is superior to the 3D instrumentation.Operative time(MD-18.41,95% CI[-37.01,0.19] P = 0.05),postoperative complications(MD1.31,95% CI[0.18,0.18,P = 0.05]),HSS score of knee joint function(MD3.46,95% CI[-2.30,9.21] p = 0.24),there was no significant difference. Conclusion: Three-dimensional printing assisted total knee arthroplasty treat osteoarthritis can reduce the amount of intraoperative bleeding, and the force line of postoperative is superior to the conventional instrumentation. It is considered that Three-dimensional printing assisted total knee arthroplasty is more effective in the surgical treatment of osteoarthritis.
