Objective: To explore the factors that affect the postoperative survival of pancreatic h ead cancer patients undergoing radical resection.Materials and Methods: The clinical pathological data and follow-up data of 111 pancreatic head cancer patients undergoing radical surgical resection from Shengjing Ho spital of China Medical University January 2012 to December 2015 were examined re trospectively.Data analysis was performed using SPSS22.0 software,and overall survi val time was described by Kaplan-Meier,expressed as median survival time,and the cutoff value was obtained by ROC curve analysis.Univariate analysis of prognosis w as performed using the Log-rank test,and multivariate analysis of COX models was performed on statistically significant single factors.Results: Of the 111 patients who underwent radical resection,71 patients with resecta ble pancreatic head cancer underwent standard pancreaticoduodenectomy,36 underwent standard pancreaticoduodenectomy with venous resection or reconstruction,and 4 pati ents Portal vein vascular replacement The median survival time of 111 patients was 12.1months.The 1-,3-,and 5-year survival rates were 52.3%,20.6% and 10.8%,resp ectively.Univariate analysis showed that age,CA19-9 level,tumor size,lymph node metastasis,tumor differentiation,tumor stage,outercapsule invasion,pancreatic margin,and postoperative chemotherapy were the factors affecting the prognosis of pancreatic cancer(P<0.05).COX multivariate analysis showed that preoperative CA19-9 levels,tumor differentiation,outer capsule invasion,lymph node metastasis,tumor size,pos toperative chemotherapy and pancreatic margin were independent predictors of prognos is in patients with pancreatic head cancer(P<0.05).Conclusion: The poor prognosis of tumor differentiation,lymph node metastasis,tumo r diameter greater than or equal to 4 cm,poor prognosis of pancreatic head cancer p atients who do not reach R0 resection,postoperative radical inflammatory combined with chemotherapy can significantly prolong the survival time of patients with pancrea tic cancer.
