Objective:To investigate the clinical significance of Mycoplasm-DNA load detected in bronchoalveolar lavage fluid of children with refractory mycoplasma pneumoniae pneumonia(RMPP)in the assessment of the condition.Methods:Selecting 98 hospitalized children diagnosed with refractory mycoplasma pneumoniae pneumonia as subjects from May to November in 2018 in the Department of Pediatric Respiratory Medicine of Shengjing Hospital affiliated to China Medical University.Bronchoscopy was performed on each subject and the alveolar lavage fluid was taken which used to detect the MP-DNA load quantitatively.According to the test results,it was divided into three groups:high-load group(>10~6/ml,28 cases),medium-load group(10~3~10~6/ml,54 cases),low-load group(?10~3/ml,16 cases).The clinical manifestations,relevant laboratory data,imaging data,and bronchoscopy findings of the children in different load groups were compared.Result:Compared with the low and medium-load groups,the C-reactive protein(mg/L)and lactate dehydrogenase(u/L)levels were higher in the high-load group;the imaging manifestation of large areas consolidation/lung atelectasis and pleural effusion were more;the fever clearance time and hospitalization days(d)were longer;nodular protuberances,mucosal erosion and necrosis,bronchial inflammatory stenosis and sputum bolt occlusion were more likely to see during bronchoscopy,and the difference was statistically significant(P<0.05).Conclusion:The MP-DNA load of children with refractory mycoplasma pneumoniae pneumonia is closely related to the clinical features.The clinical manifestations of children with higher DNA-load are more serious.It may be related to the local damage and the excessive immune inflammatory response caused by toxins and metabolic products secreted by Mycoplasma pneumoniae.The level of local MP-DNA load can guide clinicians to use drugs rationally,better predict the outcome of the disease and take preventive measures.
