Study On The Relationship Between Nutritional Status And Dietary Compliance,Psychological And Social Factors In Maintenance Dialysis Patients

Objective: To develop a scale of dietary compliance that is suitable for maintenance dialysis patients with Chinese cultural background;To understand the nutritional status of maintenance dialysis patients;To determine whether psychosocial affect the nutritional status of maintenance dialysis patients.Methods: The study was based on the literature review and the definition of dietary compliance,the framework of the scale was proposed and the item pool was formed.An initial dietary compliance scale for maintenance dialysis patients was developed through expert evaluation.General data questionnaire,used maintenance dialysis patients diet adherence scale,Self-rating depression scale,Self-rating anxiety scale and social support rating scale in China medical university first affiliated hospital of maintenance dialysis therapy 220 cases patients were investigated.To obtain the object of study of demographic data and information associated with disease,dietary compliance status,depression,anxiety and social support condition.Used SPSS20.0 to input data and statistical analysis.General information,dietary adherence,depression,anxiety and social support were described using mean and standard deviation;Analysis of the influencing factors of nutritional status by two-class logistic regression.Results: 1.The formation of the dietary compliance scale for maintenance dialysis patients The scale includes 11 items,The content validity index of the scale was 0.8-1.0.The calibration validity of the scale used the patients' mean dry weight and serum phosphorus values for nearly three consecutive months,which were negatively correlated with scale.The correlation coefficient was 0.212 and 0.244.The scale's Cronbach's ? was 0.686.Thirty patients were reassessed after a two-week interval,the correlation cofficient between the two measurements was 0.622.2.Nutritional status of maintenance dialysis patients The nutritional status of 220 maintenance dialysis patients was evaluated by MQSGA and divided into three grades.The results showed that 75 patients with normal nutrition,accounting for 34.1%;Patients with malnutrition accounted for 65.9%,and 142 patients with mild and moderate malnutrition accounted for 64.5%.There were 3 patients with severe malnutrition,accounting for 1.4%.3.Relationship between nutritional ststus and dietary compliance and psychological and social factors in maintenance dialysis patients In univariate analysis,gender,age,education,occupational status,dialysis age,dirtary compliance,depression,anxiety and social support were statistically significant for nutritional ststus.In the multivariate analysis,the patients' nuteitional status was taken as the dependent variable,and the results showed: age,dialysis age,dietary compliance,depression,anxiety and social support were the main factors affecting the nutritional status of patients on maintenance dialysis.Conclusion: 1.The reliability and validity of the dietary compliance scale of maintenance dialysis patients developed in this study is better,objective and effective evaluation of dietary compliance in maintenance dialysis patients and has good clinical applicability.2.Maintenance dialysis patients have a higher incidence of malnutrition.3.The nutritional ststus of maintenance dialysis patients is affected by many factors.Dietary compliance,psychological and social factors are the influencing factors of nutritional status.
