Objectives:To explore the value of percutaneous oxygen partial pressure monitoring in severity stratification and prognosis evaluation of patients with acute heart failure.Methods:A total of 45 patients with complete clinical data were retrospectively collected from the emergency department of the first affiliated hospital of China medical university from July 2017 to December 2017.Patients were divided into survival group and death group according to 28 days'survival situation.Clinical data of percutaneous blood gas and NICOM were collected at the time of diagnosis?0h?and 6h after treatment.Percutaneous oxygen partial pressure?PtcO2?,percutaneous carbon dioxide partial pressure?PtcCO2?,cardiac output?CO?and stroke volume?SV?were recorded at 0h and 6h,respectively.Oxygen load test values at 0h and 6h and arterial blood gas analysis values were collected.PaO2 and PtcO2,PaCO2 and PtcCO2were compared to calculate oxygen and carbon dioxide offsets.To analyze the relationship between the above indicators and the prognosis of patients.Results:There were significant differences between the survival group and the death group?P<0.05?in the oxygen load test?0h?and the carbon dioxide offset test?10min?at the time of diagnosis.After treatment?6h?there were significant differences in the indicators of CO,SV,PtcO2,PtcCO2,oxygen offset and carbon dioxide offset between the two groups;The optimal cut-off value of oxygen load value in predicting28-day death in patients with heart failure was 43.5,with a sensitivity of 93.8%and a specificity of 69.2%.The optimal cutoff value,sensitivity 84.6%,specificity 75%and area under the curve 0.853 were used to predict the death of patients within 28 days by carbon dioxide offset.Conclusions:percutaneous oxygen partial pressure can be used as a reliable indicator for severity stratification and prognosis evaluation of patients with acute heart failure.Increased carbon dioxide offset and decreased oxygen load test in 10min suggest poor prognosis,and the prognosis of patients with heart failure should be evaluated by dynamic monitoring.
