Background: Sarcoidosis is a systemic granulomatous disease with a lack of specificity in clinical manifestations.The diagnosis relies on demonstration of noncaseating granulomas and exclusion of tumor and other granulomatous diseases.The clinical manifestations of sarcoidosis and tuberculous lymphadenitis are similar,it is often difficult to differentiate sarcoidosis from tuberculous lymphadenitis,especially when caseous necrosis is not seen.The aim of our study was to screen differentially expressed MicroRNAs(miRNAs)of lymph node between sarcoidosis,tuberculous lymphadenitis and control by miRNA microarry,which were validate by quantitative real-time PCR in large samples to investigate the value of miRNAs in the diagnosis and differential diagnosis of sarcoidosis and tuberculous lymphadenitis and the correlation between miRNA and the clinical parameters of sarcoidosis.Meathod: To detect the expression of miRNAs in lymph node of patients with sarcoidosis,tuberculous lymphadenitis and control by miRNA microarray and screen differentially expressed miRNAs,which were validated by quantitative real-time PCR in larger samples.To analysis the relationship between the expression level of miRNA and clinical parameters of patients with sarcoidosis.Results: miRNA microarrays showed that seventy-five miRNAs were significantly differentially expressed between sarcoidosis and controls(P<0.05),twenty-three miRNAs were differentially expressed between tuberculous lymphadenitis and controls(P<0.05),and nineteen miRNAs were significantly differentially expressed between sarcoidosis and tuberculous lymphadenitis(P<0.05).Eight miRNAs(miR-30 b,miR-133 a,miR-145,miR-185,miR-301,miR-425-5P,miR-449 b and miR-885-5P)were validated by quantitative real-time PCR in larger samples.The results showed that miR-145,miR-185,miR-301,miR-425-5P,miR-449 b and miR-885-5P were differentially expressed between sarcoidosis and controls(P<0.05).The area under the curve(AUC)of the receiver operating characteristic curve(ROC)were 0.846(95% CI:0.735-0.957),0.840(95% CI: 0.739-0.941),0.737(95% CI: 0.604-0.869),0.756(95% CI:0.628-0.884),0.727(95% CI: 0.596-0.859)and 0.728(95%CI: 0.597-0.859),respectively.We also found that miR-145,miR-185,miR-301,miR-425-5P,miR-449 b and miR-885-5P were differentially expressed between tuberculosis lymphadenitis and controls(P<0.05),and the AUC was 0.769(95% CI: 0.642-0.895),0.752(95% CI: 0.629-0.875),0.616(95% CI: 0.472-0.760),0.724(95% CI: 0.596-0.853),0.677(95% CI:0.535-0.819)and 0.705(95%CI: 0.569-0.841),respectively.miR-185 was differentially expressed between tuberculosis lymphadenitis and sarcoidosis(P<0.05),the AUC was0.686(95% CI: 0.549-0.823),the sensitivity and specificity of miRNA-185 for diagnosis of sarcoidosis from tuberculous lymphadenitis are 61% and 80%,respectively.The relative expression level of miR-885 was positively correlated with the peripheral blood lymphocyte ratio of sarcoidosis(r=0.371,P<0.043).Conclusion: miR-185 is differentially expressed between sarcoidosis and tuberculosis lymphadenitis and can be used as adjunctive marker for differential diagnosis between sarcoidosis and tuberculous lymphadenitis.miRNAs in lymph nodes show similar expression patterns between individuals with sarcoidosis and those with tuberculous lymphadenitis,with a high diagnostic value.
