Clinical Study On Hyponatremia Induced By Pituitrin With Hemoptysis

Objective: In order to investigate the clinical features and effect factors of pituitrin treatment of hemoptysis leading to hyponatremia.And the method treatment of hyponatremia in what the use of pituitrin induced hyponatremia.Combined with the 2014 European guideline for the diagnosis and treatment of hyponatremia,and to analysis of gap between the guideline.Methods: We select 105 cases from January 2011 to July 2016 in Shanghai Jiao Tong University Renji Hospital emergency what department use of pituitrin in the treatment of hemoptysis patients.Including age,gender,statistics of pituitrin dosage,clinical data of electrolyte monitoring.Use of statistical software the incidence,clinical features and the treatment of hyponatremia in the data analysis of pituitrin induced hyponatremia,and compared with the guideline.Results: In the 105 cases of patients with pituitrin,there are 55 cases occurred hyponatremia,the incidence of 52.38%,which is associated with the cumulative dose of pituitrin was positively correlated(r=0.524,P=0.001).In the occurrence of hyponatremia,time can be acute(< 48 h,54.5%),also can be chronic(> 48 h,45.5%),and connected with the use of pituitrin(P=0.003).The multi factor analysis of hyponatremia caused by pituitrin showed that hypokalemia(P=0.000)was found in patients with hyponatremia.The average length of stay in the hyponatremia group was 5days(P=0.001)compared with that of the hyponatremia group.In the diagnosis and treatment of hyponatremia after,detection of 3 cases of urinary electrolytes,sodium supplement way is the most common oral sodium supplement(40 cases),moderate to severe hyponatremia,hypertonic saline is still the preferred emergency treatment measures.In addition,in the day of sodium supplement in a timely review of electrolytes in patients with only 5 cases,often in sodium 1-3 days after review of electrolytes.Conclusion: The incidence of hyponatremia was higher in patients with hemoptysis treated with pituitrin.It was significantly associated with the use of pituitrin,and it has specific clinical features.At present,the clinical treatment of hyponatremia induced by pituitrin is ignored.And there is a big difference between the clinical treatment method and 2014 European guidelines for the diagnosis and treatment of hyponatremia.We hope to provide a more appropriate clinical guideline for the diagnosis and treatment of hyponatremia,which can increase awareness of hyponatremia and give the best clinical guidance.
