Objective: To construct a comprehensive geriatric assessment system based on rapid screening and test the feasibility of this system, preliminary analysis of the health status of the old people.Method: Based on the comprehensive geriatric assessment, the main content of the comprehensive geriatric assessment is determined through literature analysis. The content of the comprehensive geriatric assessment is evaluated by the expert meeting, and the framework of the comprehensive geriatric assessment system based on rapid screening is preliminarily formulated to form the first round of expert letters.24 experts who met the purpose of the research were selected. After two rounds of expert consultation,a unified opinion was reached. After the discussion of the research group, the comprehensive geriatric assessment system based on rapid screening was finally constructed.300 samples were included in the hospital and community by means of convenient sampling. The questionnaire was used to measure the effective recovery rate of the questionnaire, the understanding of the elderly on the rapid screening items and the evaluation time of the system to test the evaluation system.Feasibility and analysis of the health status of the old people. Results: 1.By searching relevant literature, the content of comprehensive evaluation of the elderly was determined, including general medical assessment, physical function, mental psychology, society, environment, quality of life, and common senile syndrome.2.Initially develop a framework for the comprehensive geriatric assessment system based on rapid screening through expert meeting, including six first-level indicators and 23 second-level indicators, and set up quick screening items after each secondary indicator.3.A total of 24 experts participated in the consultation. The questionnaire recovery rates of the two rounds of experts were 92% and 100% respectively, the expert authority level was 0.88 and 0.86,the expert coordination coefficient was 0.510 and 0.768,and the approval rate of the rapid screening items was 93%,98%;finally built a comprehensive geriatric assessment system based on rapid screening, including 6 first-level indicators,26 second-level indicators, rapid screening items from 25 rapid screening questions and 5 concise scales composition.4.Through the questionnaire survey, the recovery rate of the questionnaire was 93.3%.The old people had a higher understanding of the rapid screening items. The average time for completing the rapid screening questionnaire for the comprehensive evaluation of the elderly was 10 minutes.5.The health status of the old people shows that 83.6% have problems with the disease status,88.2% have problems with common symptoms, and 69.6% have problems with the physical function status,32.1% may have mental and psychological problems,19.8% have problems with their social status, and 90% have problems with their home environment. Conclusion: This study constructed a comprehensive geriatric assessment system based on rapid screening. The indicator system consists of 6 first-level indicators and 26 second-level indicators.According to the content of each secondary indicator, rapid screening items has been set composed of 25 rapid screening questions and 5 concise scales. The construction process is scientific and reasonable, the content evaluation is comprehensive, and the results are true and reliable. This system is used to conduct an empirical survey of the old people in the community and hospitals. The results show that the elderly have a high degree of understanding of the rapid screening items, which can quickly screen out the health problems of the elderly, improve the evaluation efficiency, and fully grasp the elderly, proves that the system is feasible. This system can be used in the comprehensive geriatric assessment in the community or hospital to initially judge the health status of the old people and provide a basis for proposing targeted nursing interventions.
