Objective: The purpose of this study was to investigate the relationship between the expression of Ki-67(proliferating cell nuclear antigen)and TTF-1(thyroid transcription factor-1,TTF-1)and the prognosis of patients with non-small cell lung cancer(non-small cell lung cancer,NSCLC).In order to provide effective guidance for the diagnosis,treatment and prognosis of NSCLC.Methods: From January 2015 to December 2017,98 patients with NSCLC were treated in the Department of Thoracic and Cardiac surgery,affiliated Hospital of Qinghai University.The patients were diagnosed by histopathology(the specimens came from fiberoptic bronchoscopic biopsy,surgical resection specimens,CT-guided puncture tissue).The expression of Ki-67 and TTF-1 in NSCLC patients was detected by immunohistochemistry(Immunohistochemisty,IHC).The patients with NSCLC were divided into TTF-1 positive group and negative group,Ki-67 low expression group and high expression group.There were 45 cases in TTF-1 positive group.53 cases were negative group,42 cases were Ki-67 low expression group and 56 cases were Ki-67 high expression group.the effects of TTF-1 and Ki-67 on the prognosis of NSCLC patients were observed respectively.The data were processed by SPSS19.0 statistical software.The counting data rate was compared by chi-square test,Logistic regression was used to screen the related factors of TTF-1 and Ki-67 expression,and Spearman rank correlation was used to analyze the correlation between Ki-67 and TTF-1 expression.Analysis of different survival curves of TTF-1 and Ki-67 protein expression by Kaplan-Meier survival analysis.(P < 0.05 were statistically significant).Results:(1)In NSCLC patients,the positive expression and negative expression of TTF-1 were significantly correlated with sex,histological type,degree of differentiation and clinical stage(p < 0.05),and the expression of Ki-67 was high and low.It was related to the degree of differentiation,lymph node metastasis,smoking history and clinical stage.Logistic regression analysis showed that the influencing factors of TTF-1 expression were sex and histological type(p < 0 05),and the expression of TTF-1 in female and adenocarcinoma patients was higher than that in female and adenocarcinoma patients(p < 0.05).(2)Logistic regression analysis showed that the influencing factors of TTF-1 expression were sex and histological type(p < 0.05),and the expression of TTF-1 was higher in female and adenocarcinoma patients.The influencing factors of Ki-67 expression were degree of differentiation,lymph node metastasis and clinical stage.(3)The expression of Ki-67 was negatively correlated with that of TTF-1(r =-0.485,p = 0.000),and the expression of VEGF was significantly higher in patients with low differentiation,lymph node metastasis and lymph node metastasis than that in patients with low differentiation,lymph node metastasis and lymph node metastasis.(4)In the survival time of NSCLC patients,the survival time of TTF-1 positive expression was longer than that of negative expression.The median survival time of the two groups was 22 months and 18 months,respectively,the difference was statistically significant(p = 0.004),the survival time of high expression of Ki-67 was shorter than that of low expression,and the median survival time of the two groups was 18 months and 23 months,respectively.The difference was statistically significant(p = 0.007);(5).COX multivariate analysis suggested that TTF-1,Ki-67 and clinical stage were independent risk factors for the prognosis of NSCLC.Conclusion:(1)TTF-1 and Ki-67 are important predictors of prognosis of NSCLC,(2)There is a negative correlation between the expression of Ki-67 and TTF-1,and the expression of TTF-1 may inhibit the expression of Ki-67.(3)The positive expression of TTF-1 can be used as a benign index of prognosis of NSCLC,the high expression of Ki-67 can be used as an index of poor prognosis of NSCLC,TTF-1 and Ki-67 can be used as important reference indexes to judge the survival and prognosis of NSCLC.
