Objective:To analyze and summarize the difference between the ADC values of the adolescent and adult hepatic alveolar echinococcosis?HAE?marginal bands and the quantitative parameters of DCE-MRI,and to combine the morphological changes of the two groups to explore the diagnostic value of functional magnetic resonance imaging.Materials and Methods:A total of 41 patients were diagnosed with hepatic alveolar echinococcosis?HAE?by imaging,laboratory examination,clinical pathology and other clinical pathways,including 20 in the adolescent group and 21 in the adult group.All paients had 1.5MRI liver routine plain scan?T1WI,T2WI,DWI?and dynamic contrast enhanced magnetic examination.According to the characteristics of its composition of images the two groups of hepatic alveolar echinococcosis?HAE?were divided into two groups?where the extensive irregular T2 signal was defined as the liquefaction necrosis group,no long T2 signal was defined as the solid group?,then the corresponding image were input,Using the software to measure between groups HAE the ADC values of rim and dynamic enhanced related quantitative parameters for analysis.Results:From 20 adolescents HAE,8?40.0%?in the solid group and 12?60.0%?in the liquefaction necrosis group.The diameter of the lesion in the solid group ranged from 3.64cm to 11.43cm,and the liquefaction necrosis group ranged from 8.91cm to12.87cm,in solid group 2 cases?25.0%?is located in the left lobe of Liver,4 cases?50.0%?is located in the right lobe of Liver,2 cases?25.0%?both in the left and right lobe of Liver.In the liquefaction necrosis group,3 cases?25.0%?is located in the left lobe of Liver,7 cases?58.3%?is located in the right lobe of Liver,2 cases?16.7%?both in the left and right lobe of Liver,and 4 cases?50.0%?in the solid group had the hepatic vein and the left and right branches of the portal vein damage variously.3 cases?37.5%?had the bile duct dilated,1 case?12.5%?presented cavernous transformation of the portal vein,8 cases?66.7%?in the adolescent liquefaction necrosis group simultaneously involved the first and second hepatic portal,and 3 cases?25.0%?invaded diaphragm and gallbladder.In the solid group,2cases?25.0%?presented lower esophageal and para-aortic metastases,2?16.7%?cases of liquefaction necrosis group showed brain metastasis.Among the 21 adult patients,10 cases?47.6%?in the solid group and 11 cases?52.4%?in the liquefaction necrosis group.The diameter of the solid group was between 3.2414.35cm,and the diameter of the liquefaction necrosis group was between 3.6411.06cm.In the solid group,3 cases?30.0%?were located in the left lobe of the liver,6 cases?60.0%?were located in the right lobe of the liver,1 case?10.0%?invaded the left and right lobe of the liver,In liquefaction necrosis group,1 case?9.10%?was located in the left lobe of the liver,7 cases?63.6%?were located in the right lobe of the liver.3 cases?27.3%?invaded the left and right lobe of the liver at the same time,and 1 case?9.10%?of the necrotic group showed metastasis of the left lower lobe.The ADC value of the marginal zone of the adolescent solid group was?0.95±0.08?×10-3/mm2/s>adult solid marginal ADC value?0.88±0.11?×10-3/mm2/s,the difference was statistically significant?P=0.009<0.05?.In the adolescent liquefaction necrosis group,the ADC value of the marginal zone was?1.92±0.15?×10-3/mm2/s>ADC value of the edge of adult liquefaction necrosis group?1.44±0.07?×10-3/mm2/s,the difference was significant?P=0.002<0.05?,and the marginal zone of the adolescent solid group Ktrans,Kep,and Ve values are{?0.46±0.13,0.47±0.16?min-1,0.43±0.14},respectively>adult solidity values Ktrans,Kep and Ve in the margins of the group{?0.21±0.11,0.32±0.13?min-1,0.33±0.18},The values of Ktrans,Kep and Ve in the marginal zone of juvenile liquefaction necrosis were{?0.53±0.26,0.66±0.25?min-1,0.56±0.12}>the values of the marginal zone of adult liquefaction necrosis group{?0.37±0.27,0.43±0.32?min-1,0.48±0.12}.The Ktrans value of the marginal zone of the adolescent solid group was statistically significant compared with the Ktrans value of the adult solid marginal zone?P<0.05?.There was no significant difference in Kep and Ve values?P>0.05?.The Ktrans value of the marginal zone of the adolescent liquefaction necrosis group was The Ktrans values in the marginal zone of adult liquefaction necrosis group were statistically significant?P<0.05?.There was no significant difference in Kep and Ve values?P>0.05?.Conclusion:?1?DWI and DCE-MRI can clearly shown the morphological changes of hepatic alveolar echinococcosis clearly,especially the marginal zone.?2?Combining ADC values with quantitative parameters of DCE-MRI can better reflect the biological activity and microcirculation change characteristics of HAE in adolescents and adults of different types.
