ObjectiveTo study the therapeutic effect of minimally invasive craniotomy with small bone window on intracranial hemorrhage in basal ganglia caused by hypertension.MethodsEighty patients with hypertensive intracerebral hemorrhage in basal ganglia were admitted to our hospital from March 2017 to September 2018.Among them,40 patients with minimally invasive craniotomy through small bone window were selected as the study group,and another 40 patients with conventional craniotomy by large bone flap were selected as the control group.The two groups were analyzed in terms of intraoperative length,length of hospital stay,hematoma clearance rate,rebleeding rate and incidence of complications,GOS(Glasgow Outcome Scale)for one month and ADL(Activities of daily living)score for six months after operation.ResultsThere was no significant difference between small bone window group and large bone flap group in preoperative basic conditions(including gender,age,onset to operation time and preoperative GCS score,etc.)(P > 0.05)The average time of operation(the time between the beginning of anesthesia and the end of surgery)in small bone window group was 193.75±22.27(minutes),in large bone flap group was 232.75±38.70(minutes),in small bone window group was20.825±5.13(days)and in large bone flap group was 31.325±12.18(days).There weresignificant differences between the two groups in average length of operation(P<0.05).The incidence of postoperative rebleeding and complications(including water and electrolyte disturbance,pulmonary infection,intracranial infection)in the small bone window group was also lower than that in the large bone flap group(P < 0.05).Through the analysis of GOS and ADL scores for one month and six months after operation,it was concluded that the patients in the small bone window group had better neurological recovery and daily living ability than those in the control group.(P< 0.05).ConclusionFor patients with hypertensive intracerebral hemorrhage in basal ganglia,small bone window minimally invasive surgery has the advantages of less surgical trauma,shorter operation time,favorable for nerve function recovery and improving the quality of life after operation.It is a method worthy of clinical application and promotion.
