Objective: To compare the curative effect between the endoscopic with a small bone window in treating patients with intracerebral hemorrhage in the basal ganglion. Method: Clinical data of 57 patients with cerebral hemorrhage in the basal ganglion was colleeted, divided two groups: 1) a small bone window hematoma treatment was performed in30cases;Small bone window hematoma group of patients were under general anesthesia, cut the island, remove the hematoma, place the hematoma cavity drainage tube, Scalp was intermittent sutured, Such as high pressure can be difficult to separate the lateral fissure, then superior temporal sulcus cortex created by the drain line above operation. 2) The endoscopic was performed in 27cases, the group of patients were under general anesthesia, Spying row needle through the hematoma, then pull out the needle, slowly rotate through the import channel endoscopic puncture and guide sheath, Repeated washing and sucting in the hematoma cavity, Slight oozing with normal saline repeated washing, In case of active bleeding is a clear vision after repeated washing to electrocautery to stop bleeding. It is no hesitated that to craniotomy immediately if bleeding, then remove the hematoma, Repair bone defects in the two groups were neither. Patients were given vital signs monitoring and bleeding, only the acid, anti-inflammatory and other symptomatic treatment. Result: Small bone window hematoma hematoma with endoscopic group in sex ratios, admission GCS score, age distribution(P>0.05) no significant difference, In the postoperative infection rates, rebleeding rates, the incidence of cerebrospinal fluid leakage on (P>0.05) no significant difference, In the hematoma residue, hematoma statistical comparison rate (P<0.05) endoscopic hematoma has an advantage, In the final results, both of them are no clear difference. Conclusion: a small bone window with endoscopic in the basal ganglia of the treatment without a clear difference. . With the equipment, such as improved surgical proficiency, endoscopic surgery is an effective surgical method inthe removal of the hematoma.
