Surgical Outcome And Analysis Of Prognostic Factors Of In Medically Refractory Temporal Lobe Epilepsy

Objective:To evaluate the prognostic factors of surgical operation in patients with refractory temporal lobe epilepsy.Methods:The clinical data of 50 patients with refractory temporal lobe epilepsy who underwent surgical treatment at the Department of Functional Neurosurgery,Brain Hospital Affiliated to Nanjing Medical University between March 2009 and March2018,were analyzed retrospectively.All patients underwent postoperative follow-up over 1 year.According to the follow-up results,Engel class?was taken as the favorable prognosis group and Engel class?-?was taken as the unfavorable prognosis group.Possible prognostic factors,including onset age,age at operation,course of operation,seizure frequency,positive or not on MRI,consistency of non-invasive diagnostic methods and positive or not on pathology,were statistically analyzed by t-test or Chi-square test.Results:36(36/50)patients were in the favorable prognosis group,14(14/50)patients were in the unfavorable prognosis group,among them,5 were Engel class?,5were Engel class?and 4 were Engel class?.Consistency of non-invasive diagnostic methods(?~2=8.747,P=0.003)were statistically significant.Onset age,age at operation,course of operation,seizure frequency,positive or not on MRI and positive or not on pathology were not statistically significant(P?0.05).Conclusions:The surgical outcome of refractory temporal lobe epilepsy was favorable(72?).The operative prognosis of the patients with refractory temporal lobe epilepsy are associated with consistency of MRI and V-EEG.The patients who were diagnosed as refractory temporal lobe epilepsy need to obtain preoperative evaluation at a early age.
