| Objective Retrospective analysis of the efficacy and toxicity of nasopharyngeal carcinoma treated with cisplatin or nedaplatin combined with concurrent radiotherapy. Methods A total of 106 cases of locally advanced nasopharyngeal carcinoma patients treated with cisplatin or nedaplatin combined with concurrent radiotherapy were collected from July 2012 to January 2017 in the Department of Radiation Oncology,General Hospital of Ning Xia Medical University. There were 47 patients in the cisplatin group and 59 patients in the nedaplatin group. The general data of the two groups such as age and dose of chemotherapy in the concurrent chemoradiotherapy were compared by the rank sum test. The general data such as gender, stage,KPS score and EB virus were compared by chi-square test. The Kaplan-Meier method was used to compare the overall survival rate, local recurrence-free survival rate, and distant metastasis survival rate between the two groups. The severity of adverse reactions which compared between the two groups using a rank sum test, such as skin and mucous membrane reactions, nausea and vomiting, hematological toxicity, liver and kidney dysfunction. Results The median follow-up of 106 patients was 59 months(12-76 months),and no follow-up was lost. The follow-up rate was 100%.Among them,9 patients(19.15%)died in the cisplatin group,8 patients(17.02%)had distant metastasis, and 2 patients(4.26%)had local recurrence. There were 11 patients(18.64%)died in the nedaplatin group, and 9 patients(15.25%)had distant metastasis. There was no local recurrence in the nedaplatin group. The OS of cisplatin group at 1,3,and 5 years were 97.9%,89.1%,and 78.0%,respectively;DMFS was 100.0%,91.2%,and 82.4%,respectively;LRFS was 97.9%,95.7%,and 95.7%,respectively.The 1,3,and 5 years OS of the nedaplatin group were 98.3%,89.8%,and 80.2%,respectively;the DMFS were 100.0%,93.1%,and 81.8%,respectively;the LRFS was 100.0%,100.0%,and 100.0%,respectively.There were no significant differences in OS,LRFS,and DMFS between the two groups at 1,3,and 5 years(P<0.05).Compared with the cisplatin group, oral mucosal reaction, nausea and vomiting were significantly lower in the nedaplatin group, the difference was statistically significant(P<0.05).There was no significant difference in leukocytes, hemoglobin, platelets, neutrophils decreased, skin reactions, salivary gland reaction, liver and kidney function damage in the two groups(P>0.05).Conclusions Cisplatin combined with concurrent radiotherapy and nedaplatin combined with concurrent radiotherapy in the treatment of nasopharyngeal carcinoma is similar, but the patient’s oral mucosal reaction, nausea and vomiting response after Nidaplatin combined with concurrent radiotherapy are mild, and the patient’s tolerance is high. It’s worth clinical promotion. |
