Objectives:To observe The Clinical Effect Observation of Percutaneous Transforaminal Endoscopic Decompression combined with Needle Knife in the treatment of Lumber lateral recess stenosis and to provide new ideas for the treatment of lateral stenosis stenosis.Methods:Among patients with Lumber lateral recess stenosis who were admitted to the Department of Orthopaedics from December 2017 to December 1818,90 patients who met the diagnostic criteria and included the criteria were selected.According to the random order method,they were divided into group A,B and group C,30 cases each.Group A: Percutaneous Transforaminal Endoscopic Decompression combined with Needle Knife release treatment;group B: Percutaneous Transforaminal Endoscopic Decompression;group C: Needle Knife release treatment;intervertebral foramen treatment 1 time,needle knife release weekly treatment 1 Third,3 weeks for a course of treatment;3 groups were given anti-inflammatory,analgesic,nutritional nerves,bed rest,and guided functional exercise treatment.The visual pain simulation score(VAS)was used to evaluate the patient's low back pain and the dysfunction index(Oswesry Disability Indsx,ODI)to assess the quality of life of patients before and after the end of the treatment.The patients were followed up for one month,and the improvement of the sagittal diameter of the crypt was evaluated by CT/MRI.The postoperative clinical efficacy was evaluated by the modified Macnab efficacy evaluation standard.The above data were statistically processed using the SPSS 20.0 software package.Results:1.90 cases were actually completed,including 30 cases in group A,30 cases in group B,and 30 cases in group C.There were no significant differences in gender,age,duration of disease and segment distribution between the two groups(P>0.05).Comparable.2.One week after the end of the treatment,the postoperative VAS score and dysfunction index(ODI,including each item and total score)were significantly lower than those before surgery(P<0.05),and the difference was significant,indicating three treatment methods.Both can significantly improve the clinical symptoms of lumber lateral recess stenosis.Compared with the three groups,the VAS score and ODI index of group A were significantly lower than those of group B and C(P<0.05).The difference was statistically significant,indicating that intervertebral foramen combined with needle knife release treatment The stenosis of the spinal canal stenosis can more effectively alleviate the pain of the lower back and improve the quality of life.The overall efficacy analysis is as follows: the total effective rate of group A and group B is 93.3%,80.0%,and that of group C is 75.6%.Group A>B >C;the efficacy of the three groups were compared between groups,P<0.05,the difference was statistically significant.It is indicated that the treatment of lateral crypt stenosis of spinal canal stenosis with intervertebral foramen combined with acupuncture can achieve better curative effect.3.The patients were followed up for 3 months after the end of the treatment.The CT/MRI examination showed that the sagittal diameter of the crypt was compared with that before operation.The A and B groups were effectively enlarged,and the changes were statistically significant(P<0.05).No significant changes were observed and the changes were not statistically significant(P>0.05).It shows that the Percutaneous Transforaminal Endoscopic Decompression is superior to the needle knife release in expanding the sagittal diameter of the lateral crypt.Compared with the three groups,the sagittal diameter of the lateral crypt of group A was more significant than that of group B and C(P less than 0.05),and the difference was statistically significant.It is concluded that the Percutaneous Transforaminal Endoscopic Decompression with needle knife release has the advantage in enlarging the sagittal diameter of lateral recess.Conclusions:the Percutaneous Transforaminal Endoscopic Decompression with needle knife can effectively improve the clinical symptoms of patients with Lumber lateral recess stenosis and improve the quality of life,which is worthy of clinical promotion.
